The Partially Destroyed Diary item for the All Sides of the Story achievement can be found in the Waking Shores at the coordinates 23, 60. To reach the bookcase where the book is located, you need to go into the Vaults at 27, 61, also in the Waking Shores. Head down the stairs, and then take your first left. Continue until there's a room on your left side again, and enter that room. On the right side, there's a bookcase with an interactable book which starts the Memories Revived quest! I also submitted a screenshot of the book in the bookcase, hope it helps!

You need to be level 68 to start the quest. However, you can loot the diary before reaching 68.

i"m lvl 70 for a while now and when i wanted to turn this in the qst ? there but....Veritistrasz is nowhere to be seen and some guy Voraxian is standing there instead.
He gives you some chat and turns into a dragon and flies off.
Should i abandon the qst or?

i lost track of this after getting the time capsule. if your wondering why its not in your q log its because you havnt got the diary in the obsidian citidel yet. it should pop up on your map once you have it

If you cannot find Veritistrasz to turn in the quest at Ruby Lifeshrine, thenyou have a different quest that involves him somewhere. You will have to finish the other quest that you have with him involved then go back and turn it in to him at the original location. When I turned in my different quest, he did not move so I sat around for a bit and finally went back to check and he was back in the correct spot to turn this in... but I had to finish the other quest. (You can probably drop that other quest if you really want to turn this one in, but then you chance getting lost on that chain so probably better to just finish up that other quest!)
