Reward for completing the new to 8.3 Blackrock Depths Pet Battle Dungeon Challenge. After winning the final battle talk to Tasha Riley and pick the first dialog option, to join them and this spiffy title shall be yours.

There are currently two ways to obtain this account-wide title:

Complete the Blackrock Depths pet battle dungeon on either mode. Before leaving the dungeon, speak to Tasha Riley and select the dialog option "Join COUNCIL OF CHAOS!"
OR collect the three pieces of the battle pet Carpal.

As far as I know, this is the only title in WoW with two completely different ways to obtain it. Thanks to all who commented information about Carpal!

How to obtain this title via Carpal

Purchase Animated Radius for 250 Polished Pet Charm in Maldraxxus or Oribos.
Obtain Animated Ulna, which is a very rare reward from Maldraxxus battle pet world quests. The world quests will indicate on hover if they reward it.
After completing steps 1 and 2, loot Flexing Phalanges from Skeletal Hand Fragments /way 47.39 62.11 Skeletal Hand Fragments
Use any of the three items to assemble Carpal.
You will probably receive the title after a delay of several minutes.

Steps 1 and 2 can be completed in any order, but you will be unable to collect the third piece until you complete steps 1 and 2. Carpal can be created by any covenant, and there are no known reputation requirements.

You can earn the title on your very first completion of the BRD pet battle dungeon (i.e. normal mode: reviving and bandaging pets are allowed), but you must speak to Tasha before leaving.
If you forget to get the title on your normal mode run, you can still get the title from Tasha on another run, but you will have to complete the dungeon on challenge mode (reviving and bandages are not allowed). You can only complete normal mode once.
Tasha also gives the option to "refuse," instead of joining. Selecting "refuse" has no effect whatsoever. You can immediately re-open the dialog box again and select "Join COUNCIL OF CHAOS!" to get the title.

Prerequisites:

The only requirements to enter the BRD pet battle dungeon are one level 25 battle pet for normal mode, fifteen level 25 battle pets for challenge mode.
However, even on normal mode most of the BRD pet battles are extremely challenging at the time of writing, more so than the other dungeons. You will not be able to complete normal mode without at least ~twenty level 25 pets, assuming you have the specific pets (and for some strategies, the specific pet breeds) required.
The other pet battle dungeons are not prerequisites to enter BRD, but BRD continues the story from the other dungeons.

Just finished whole dungeon on normal mode and never got dialogue to choose a title from Tasha last thing npc said was: ''join our squad'' and thats it, after that i couldnt choose anything, tried to reload still nothing. So by this either is a bug or doesn't work on normal mode anymore

Just a warning - this can definitely still be bugged. Completed this on normal for the first time last week just for this title, Tasha Riley disappeared after the last battle. You could hear her dialogue but no NPC. I tried relogging, which made her reappear but she gave no option to join the squad. Opened a ticket but GMs insist they can't verify it's completed despite my having done the Burt Macklyn quest - which I had to hearthstone and fly all the way back to Blackrock to hand in because the gates also didn't open at the end of the dungeon. Gutted as the only way I can get the title now is doing challenge mode and I don't think I'm up to that. Especially since several of the normal strategies don't work or are much more RNG with the change in pet abilities.

So, I just made Carpal, the pet where you combine WQ reward, Undying Army item, and the Phalanges on the ground ... and it gave me this title. I've never stepped foot in a pet battle dungeon. Not sure if it's a bug or what. Anyone else have this happen? Added a screenshot to the gallery.

I can confirm as of 1/19/2021 the title is obtainable again, and is awarded retroactively.

I just got the title from defeating Rotgut, while already having the Animated Ulna in my inventory, failing the WQ in Maldraxxus.

I also got it from crating Carpal.
I got it about 15 mins after crating him. Was running arround in the Conservatory and the title just came out of the blue.

Came to this page wondering why I was awarded this title, because I seemingly got it out of the blue while sitting in my Covenant Hall.
But yes, I had crafted Carpal about 10 or 15 minutes prior to that.

As of today (Feb 5, 2022) creating the battle pet "Carpal" is still giving the Minion of Mayhem title. I got it about 15 minutes after combining the parts to make the pet tonight.

I just made "Carpal" the 6th of december 2022 and i did not get the title so seems like they removed it from making Carpal in Dragonflight

I suspect the issue with people not getting the dialogue option for the title (I've ran this once on normal and twice on hard to no avail) is because during stage 5 we get 2 of 3 random battles and the 3rd one stays locked behind a gate and is bugging out the "completion" flag that opens up that dialogue. I have no knowledge if this is why but it's the only logical thing I can think of.

So, I tried 6 times in the first week to get it done - first on Normal mode, then again, then on Normal mode after abandoning the quest, then Normal again with the quest retaken, and then Challenge and then Challenge again with the quest completed. None of them worked.

I put in a few tickets, none of which got a helpful response. So I tried again today in the new reset week and it worked - I could interact with her and get the title. I spoke to her BEFORE all of her dialogue had been finished, but I don't think that matters since the first time I did it I did that too and it didn't give it to me. I could also still see the un-battled pets behind the doors and the Dungeon Finder "eye" icon still says "In Progress" so I have no idea why it let me do it this time. I'd guess it's just luck.

Oh, and for the record I fought Ralf, then out came Templeton and Splint. The only thing I did differently was I fought Splint first, who was on the left side. Usually I always fought the right side pet first, who in this case would have been Templeton.

I hope some nugget of information is here helps anyone who tries to get this in the future. My best advice would be to keep trying each week and to slightly change up what order you fight the randomized pets. Not sure why it worked for me this time but I'm just thankful it finally did.

5 resets in of always killing the left pet first, will update you when the title option finally becomes available from Tasha Riley.
