Just in case anyone else comes here looking for this info:
As of the date of my posting, after you kill the enemy you first made contact with in battle the next enemy you make contact with becomes your "adversary". Meaning there will always be ONE enemy with this debuff on them throughout your time in combat. This is most poignant regarding questing or dungeons of any level, as for raids you will likely hit the boss first (as you likely should), so they would have the debuff on them for the duration of the boss fight.

EDIT: I cannot speak to PVP. I haven't PVP'd in a long time.
