Actually this quest is really slower on Beta, because just have two Pain Conduits and you need wait respawn.

Pain Conduits locations:

/way 31,0, 15,1
/way 26,9, 14,6

Grathalax is located in the underground of the "The Tremaculum".

/way 31,6, 17,0 (grathalax)

You find him underground, taking the entrance near Malevolent Stygia's cave, in The Tremaculum

To complete this quest you need to disable 3 Pain Conduits and defeat Grathalax.

Pain Conduit #1 can be located at 31.01 15.11 coordinates.
To locate and disable Pain Conduit #2 go to 26.87 14.66 coordinates.
Travel to 24.57 17.29 coordinates to disable Pain Conduit #3.
Grathalax can be found in the underground hall, entrance is at 27.84 20.37

There is also Torture Implements next to the wall behind Grathalax.
Open it to loot Box of Torments and start Box of Torments (150 rep with Ve'nari)

If someone could make a video showing how to get from the pain conduits area of the Tremaculum to the entrance of whatever cave leads into the Extractor's Sanatorium, that would be GREAT. I explored ten ways from sunday but couldn't find one, even with the advice from other comments, so I ended up with no choice but to jump down into the area from above & use a glider to land safely.

For those being very careful on Eye of Jailer aggro to maximise stygia and reputation gain, be very mindful of this weekly because it increase Eye of Jailer aggro while partially done as compared to other quest which only gain increment when fully complete.

Once you got all 3 conduits, you will gain roughly 0.4 bar of eye of jailor, and another equivalent amount after killing Grathalax. It's crucial for those that doing this quest last and doesnt have much spare eye of jailer to aggro.

Defeat Grathalax underneath the Tremaculum in The Maw.

Description

There are many powerful lieutenants among the Mawsworn, but there is one who is particularly important to the Jailer. Far beneath the Tremaculum lives the Jailer's prime interrogator, Grathalax. Power is fed to him via conduits throughout the area. His role is to make the process of anima extraction more efficient. His methods are... unkind. I believe we would both benefit from the end of his existence.
