If you join the fight after the initial dialog, the quest won't trigger properly. It'll play out silently and you won't get the credit. Just wait 45 seconds or so for the respawn and attack it fresh, then it'll work.

Been stuck on this since yesterday filled with people and Usurper Simona is stuck and is orange unattackable tried to retake the quest turn off all my addons doesn't help

Ive noticed that if u switch servers u might find her not bugged. I Joined an random group and got into an other server where she was not bugged.

I did not get credit a few times. When I did get credit I did not attack from stealth (did on rogue) and I never left the circle area on the minimap. In other words I stayed right next to her and KT. I dont know if that is what caused it to work but I figure sharing the information could not hurt.

Just failed this quest twice by dying without taking damage after the Usurper had turned orange. Third time I made sure to not have any dots on her at ~50% and it worked. Or it was completely random.

She is on the same "level" as the questgivers, standing at a ledge.

From questgivers, go to
/way 42.63, 52,52

That's the "entrance" to the ledge she's on. From there go forward to
/way 41.07, 54.70

At this day, (9.1 already released) the PNJ still can be bugged. Just for information...
Try to switch server to unstuck your progression...

6/8/22 and it still bugs.

I killed her 20 times at least. I swapped to multiple other servers. I logged out. Dropped quest and picked back up. I tried at different distances. Nothing, Wouldn't give me credit.

Then I dropped the quest again and logged out of the game. I came back in, accepted quest, "killed" her and Kael'thas spoke and it gave me credit.

So there's still something a bit off with this quest. It bugs for some people.

So for this quest, don't kill her, don't use CDs on her. Bring her HP down slowly until the accuser appears near 50% of her health.
