The portals are not in the building they are all out in the fields right behind and adjacent to where you spawn into the vision.

Edit: Preventative Measures is one of the daily mini-Horrific Vision quests offered in the zone under assault by the Black Empire (Uldum or Vale of the Eternal Blossoms). The quest location is shown on the zone map and looks like a traffic light. You can do the quest before you do the assault (or other quests) since the mobs you kill don't count toward the assault. Next to the quest giver is a red frame holding eyeballs - this is the entrance to the mini-vision (and exit).

If Uldum is under assault, the portals you must close in the quest are in N'Zoth's alternate reality version of the Vir'naal Oasis. The portals are within the walls of the city. If the Vale is under assault, the portals are in a large grassy area at the foot of N'Zoth's version of the Shrine of the Seven Stars.
The purpose of the quest is to earn Corrupted Mementos and Coalescing Visions. They're part of the quest reward. Both also drop from some mobs in the quest area, particularly mini-bosses. Corrupted Mementos are used in the Chamber of Heart to upgrade your Titanic Research Archive. The Archive consists of talent trees that you build. The "talents" provide abilities that make it easier to complete the Horrific Visions given by Wrathion and upgrade the legendary cloak, Ashjra'kamas, Shroud of Resolve.
Coalescing Visions are used to buy the Vessel of Horrific Visions from Wrathion which is your ticket to enter the horrific vision from the Chamber of Heart.
During the mini-vision do as much as you can before your sanity bar goes to zero. In the Vale, extend sanity by freeing frightened Pandarens (many are in the Shrine up the the curved stairs). In Uldum, free the citizens in the Vir'naal Oasis. The extra sanity is shown as a gold ball. You can keep moving while the sanity buff is being cast on you.
The vision may be entered once a day. The daily quest will vary.

Don’t bother with the small purple void portals scattered around the Assault area. The portals are inside the vision.
The “Horrific Vision” is over-hyped. Where is the “horrific” in the vision? There are many large round portals in the vision that are closely spaced. When you click on a portal, no mobs emerge.

To enter lesser visions you need to have completed the 8.3 Black Empire Campaign which begins with An Unwelcome Advisor, or at least progressed through Descending Into Madness.

Description

Waves of darkness continue their assault on the shores of our world. I am afraid the waters at our feet may soon wash over us. Perhaps it is time for a different approach. Make your way into the twisted realm and seek out their dark portals. If we are able to slow their advance it might just buy us the time needed to expose a fatal weakness.
