I've been running through the dungeon a lot on PTR, and trying to find out exactly what spawns him. Here's what I've found so far, keep in mind this is on PTR and may change.

Some notes on Fras Siabi:

During Stage 7, behind the Risen Guard, Ezra Grimm will spawn with the world quest Angry Residents. This rewards Cleansed Remains x1.
Ezra Grimms spawn conditions are completely RNG. He is not always up for everyone at once, nor is he guaranteed to be up. I've done the dungeon approximately 8 times fully (without turning in the quest) and he turned up twice. Those two times were within the same reset period.
Due to the reasons above, it is possible to make Ezra Grimm spawn, and get the optional world quest every week. This can be time consuming, and I'm not sure if its worth it. To do this, do the dungeon up to stage 7, and if Ezra Grimm is not present, leave the instance. Keep repeating this until you get him.
Postmaster Malown may spawn under similar circumstances, however has not been found. He may have a world quest tied to him, or special circumstances. There is currently an interactable letterbox during Stage 7 which may be relevant.

Since it takes about 15 minutes to get to Stage 7, I feel it might be worth it to reset the dungeon to get Fras Siabi to spawn if you want to collect all the rewards in as few weeks as possible. You can purchase all four pets in 22 days if you gather Cleansed Remains x2 each reset and start on a Monday. Unfortunately, these pets cannot be caged, so speeding up the process doesn't have a lot of benefits. Although, if you use your currency to buy Unopened Stratholme Supply Crate instead, you will get extra Polished Pet Charms which you can use to buy other BfA pets that can be caged.

Keep in mind, this world quest is NOT account-wide! People who have enough alts (who can enter this pet battle dungeon) could get all the tokens they need really "fast", the only limiting factors are the number of characters you can bring and the time you want to spend there with all the resets and clearing 6 stages to see Fras Siabi and the WQ on all of these characters.

And thinking about it, the weird spawns make sense and seem to be intended... it only happens to slow down / demotivate hardcore token grinders. I guess the "casual" player is meant to only do a full clear once a week and to be happy if he gets a 2nd token because WQ is active in that run.
