Plaguefallen Chest treasure in the Maldraxxus zone
It is on top of the wall near the Palgue Watch windrider

To get the treasure you need to

Go to the Scunner rare area
There is a tunnel to the sewers under the platform
/way 62.40 76.56 sewers tunnel
After you find a tunnel you need to get the Plaguefallen debuff by standing in toxic green pool
After you get 10 stacks of Concentrated Plague debuff you'll be transformed into a Plaguefallen
You have to reach the chest in 2 minutes (But your speed decreases because of the debuf)
Can just eat any food that restores high amount of hp and get all 10 stacks
Then you need to go into sewers
And there is a pipe at the end of the tunnel.
Use the pipe and you'll get to the chest

Reward is Iridescent Ooze

Criteria of: Treasures of Maldraxxus

The chest is on the wall, you need to use Pipe to rech it. The pipe is in a tunnel, entrance is at 62.38 76.52.
Chest requires you to be debuffed by Plaguefallen spell.
To get it keep standing in a green slime all around the area until you get 10 stacks of Concentrated Plague.
After that morph into Plaguefallen form. Standing in the silme is a pain, should use any healing stuff.
After getting the debuff follow into the tunnel and use pipe to be transfered on to the wall and open the chest.

How to get Plaguefallen Chest:

go to /way 62,6, 76,5 and touch on the green water until reach 10x Concentrated Plague stacks to turn into Plaguefallen
with Plaguefallen enter the cave at /way 62,3, 76,6 and interact with Pipe
after you teleported open the Plaguefallen Chest at /way 57,7, 75,9

As of Sept. 20 on Torghast beta server there's no pipe to interact in the cave, so don't bother.

There are Plaguefallen Potions on the ground which immediately transform you into the Plaguefallen instead of trying to survive 10 stacks. Furthermore, it might require level 60.

With the help of double jump and Infernal Strike DH's can get to the chest without using the pipe and all that. However at the time of this post neither the chest nor the pipe are appearing in the game.

As of live, there is no longer a tunnel. It is just a cave with walls and nothing intractable.

You need level 60 and the Plaguefallen debuff to interact with the pipe in the small cave besides Scunner, confirmed this myself after trying at level 56 and then again at 60.

You can get the Plaguefallen debuff by intentionally taking damage in the slime lake/slime pools, or drinking a Plaguefallen Potion and walking from the camp (where the pots usually spawn) to the cave.

Didn't know there was a pipe and hearthed to the maldraxxus hall and used the portal to get to the top of the primus then glidered down.

Good news,

As of 28/11/20 this is fixed, the pipe entrance is now present where it should be and the chest is also present. The chest REQUIRES to be afflicted by the plaguefallen debuff to open, so no heroic leaping/etc. you need to get it "properly".

If there´s any rogue trying to do this, what i´ve done was, with "cheat death" talented, at 5 stacks, drink ur heath pot and simply wait to get all 10 stacks, the extre 3 seconds from "cheat death" is just enough to transform. Im sorry about my english if there´s any typing mistake. Hope this helps someone xD

Big brain tactic -
1) Get Flytrap abomination;
2) Drink the Plaguefallen Potion from the tables;
3) "Mount" Flytrap
4) Run to Scunner boss area, below it (behind) there's a small cave with clickable pipe
5) Dismount Flytrap, get into pipe.
6) Voila, you are at the top of the wall and can loot chest.

Warriors can still use Heroic Leap with the Plaguefallen debuff to make the trip to the pipe a bit faster.

Soulshape works after drinking a plaguefallen potion. Little extra speed boost to get you to the cave faster.

Once your are at the top of the waterfall through the pipe - the slime water there will STILL DAMAGE YOU. And if it kills you - there is no way to get back up there to get your corpse. You have to release at spirit healer :|

Edits: You have to be 60. EASY directions for whoever wants to get this washout having to read all the other unnecessary information. If you're not a healer, get some kind of out of combat healing food. In Madraxxus, go to
/way 62.40 76.56 sewers tunnel
This is under the large platform. You should be able to see the entrance of a cave.
Stand anywhere in the green water to get 10 debuffs of whatever and then you'll be transformed into a green skeleton.
Walk into the cave, open that small door. You'll be ported somewhere. Now walk over to the chest and grab it. Done.

I wanted to come here and say getting to the chest was surprisingly easy to do once I figured out that you could turn into an actual plaguefallen with a Plaguefallen Potion found on the ground at Mad Mortimer at 59, 71. I wasted so much time DYING trying to get the 10 stacks of Concentrated Plague using foods, potions, my crimson vial, etc.. in the green water.

Being a rogue helped as I was able to stealth past a Colossal Goo, grapple and use my sprint to get to the pipe before the 2 minutes was up.

ALSO, do not waste your time trying to grapple or scale wall to get to the chest, bc it will be locked unless you are in the Plaguefallen form to unlock it!

In yet another shockingly brilliant example of development, they've made it impossible to retrieve your corpse if you die up there as a result of the poison stacks. As always - I am beside myself with shock and awe at the intelligent design here.

You don't need to kill you self.
Beside of the green people; Plague Inventor there is green portion: Plaguefallen Potion. around 59:72 Thay are on the ground Drink to you going to look like a plague man. Then you have 2 min to get to the drain. 62:75 under the platform ... :-)

Lots of justified complaints on how to get back down if you don't have a Goblin Glider, slow fall, portal, or whatever. For those of us who don't, here's a fairly quick workaround.

Hearth to Dalaran.
Use the proper portal, in Greyfang Enclave (Alliance) or Windrunner's Sanctuary (Horde), to get back to your home city's portal room.
In the portal room, use the portal there to get to Oribos.

In case you thought flying would allow cheesing of this chest, it doesn't. The buff is needed to unlock it and you can't mount while plaguefallen.

Possible "cheat": if the game crashes, the debuff may go to 2min again (it didn't work with log off). Though it's not important especially if a class has speed boosts.

If you are a Warlock with flight, you can fly up to the chest at 57, 75 and use Demonic Circle. After that, fly down to the mushrooms close to the wall and use your Demonic Gateway to the ground. Fly down and stay on the lake while using Drain Life on any ooze so you don't die. Once you get the buff, use the portal and the circle and you have it super easy!

By accident I had Unity with me when I tried this. It is possible to mount Unity (probably any other construct) and be much faster than walking. So drink the potion (there are at least two of them) on the platform with the NPCs and the mount your construct. Easy peasy!

If you die at the top of the wall, once in spirit form go left to bleak redoubt. Go up to the butcher's block and once there to the left of the stairs is a gold mushroom looking thing with what looks like a trail behind it. You can easily go up this in spirit form and drop down to the water behind the wall, jump up on wall and rez. I'll put a screenshot of the trail up.

Went to /way 62.38 76.52 Entrance to the tunnel. Dropped a palatable feast on edge of goo. Sat in goo and ate until 10 stacks, offsetting damage. Went in cave to pipe. Walked to chest.

1. get plaguefallen potion on table behind npc giving quests.
2. Use speed thingys to leg it to the pipe...like druid travel form
3. get chest and epix...no need to kill yourself in slime

It looks like this treasure is unattainable when Scunner is up. The potion and the buff both work perfectly fine, but the pipe does not appear to spawn while Scunner is on his platform.

If you have a friend druid who has Lunarwing (Legion owl flight form) form unlocked, then your friend can fly you there and you don't need to click the pipe. You will turn back into human form while mounted on your friend, but no worries since it's only because those Plaguefallen slime boys don't have mounted animation - the debuff will remain on you.

Intuitively it should work for other mounts that allow you to carry party members too, but I forgot to test it with my friend and now I'm too lazy for it.

The best way I found was to step into the water and get into a pet battle with the NPC at 61 78.
Wait to collect all stacks and leave the battle.
Go to the cave at 62 76 and enter the pipe, then go to the chest and open it.

Couple easy ways for certain classes to get this with flying
Mage Alter Time Fly to chest, cast it, quickly fly down and drink, teleport back
Monk Transcendence Fly to chest, cast it, quickly fly down and drink, teleport back

Paladin/Engineers, Blessing of Freedom Works when effected making it easy to get to the pipe, also Nitro Boosts

Thanks to Varenne for coords of cave /way 62.40 76.56 sewers tunnel - for any without coords - the cave is quite a distance from the chest.

When facing the chest, take a look behind you to the next wall/rampart along and hover your mouse over the little triangular bump; it says Pipe - this is where you will come back to after turning yourself into a slime.
To get to the tunnel, fly east, slightly south (4o'clock-ish), you'll see a round platform in the distance, fly across that and duck under the edge, there's a pool of slime water and a small round cave tunnel right next to it.
All advice about sitting in the water eating but keeping food and/or healing up until you get to 10 points and turn into a plague man is perfect.
As soon as you change, run into the tunnel, the plaguefallen pipe is close and highlighted, click it and you'll be back up on the wall one over from the chest. Keep moving! :)

Find yourself a pair of these: regurgitated kyrian wings

Stand in the dirty water right underneath and heal yourself as best you can.
If you manage to get past 10 stacks you will become plaguefallen and you can open the chest.
You just can't fly, therefore the wings are perfect, shoot you right up to the area and you can open the chest with no issue.

Just a heads up, but if you decide to get the "free no-risk potion buff" instead of the "super-risky get 10 stacks or die" one close by, then you can re-apply the buff by drinking any other potion in the area (there's a strength, and a health one for example) which may save you precious seconds.

Or, just do the "super risky get 10 stacks or die" buff, and spam eating tasty seraph karagi so the 5 secon heal proc staves off extra damage while you wait for that 1oth stack. Also, if you have any kind of sheild bubble, or heal-over-time, hit that before collecting stacks coz any action other than eating will reset the counter.

Once you have slimed up, you should start (or continue) to heal as you head towards the pipe and the prize. You are awarded with a pet, a laughable amount of anima, and some trash. Yay...

you will get the buff Plaguefallen for 2 min
walk to the Plaguefallen Pipe at 61.6 76.7 and click it
walk south over the green river (60.2 75.5)
walk south east to the circle shaped plattform (62.1 75.9)
Jump off the platform's south edge (62.6 76.8)
Go west into the cave
you appear at the wall at 57.7 77.3. The buff is reset to 2 min.
