To reach this treasure you need interact with the teleport Translocation Sphere: House of Plagues and skip the mobs until reach the treasure.
The treasure is located at the end of the House of Plagues (you must go up an elevation to the left).

Hard to find House of Plagues:
To reach this treasure you need interact with the teleport Translocation Sphere: House of Plagues and skip the mobs until reach the treasure.
I'm not find a good path to reach this treasure (sorry), so I just use my Harpoon in a mob on the top near the treasure.

Just got a Kevin's Party Supplies toy along with a Plague-Etched Hacker from a Slime-Coated Crate at /way 65.89 88.31.
Toy is not 100% as I loot this treasure daily after I hit 60 and only got it at the 4th attempt, 2 times at that location, and a couple times with the crate at different location, just look for this treasure in your map daily and you will get it!

Is there a prerequisite for these? I checked each (as a level 60) and there wasn't a chest at any of the locations.

To get to the one at 65, 82, start at the building in the back of the slime-collecting area, at 62.4, 79.7. Facing the building, I found 2 easy-ish options.

Important note: Both involve swimming through slime, which applies a stack of Concentrated Plague every 2-3 seconds that you're in it. You also get a Caustic Fog debuff while in the area, which stacks every 20 seconds, up to 9 stacks. Caustic Fog increases the damage of Concentrated Plague, and that damage can ramp up fast, so you want to be quick. Jumping through slime does NOT prevent or slow Concentrated Plague stacks; with unlucky ticks it can actually stack faster.

Option 1 - Turn left and follow the trail of yellow grass down the hill, until you get to the two hollow glowy-veined mushrooms. Go between those mushrooms and the huge tree-mushrooms and walk straight along the paved ledge against the wall. Stop at the corner.

Option 2 - Go around the building to the back-right corner, up onto the hill there. Follow the ridge up to a point where you can jump onto and over the wall. Once you're on the other side, go left and jump up onto the paved ledge against the wall (the slope is a little finnicky, just stick close to the wall). Follow the ledge and stop at the corner.

From here, the way to get to the platform is the same. Jump into the slime, swim to the rocks around 65, 80.6, and use them to jump up onto the platform. There's an underwater ledge on the right-ish side of the rocks that makes it easier to get up on that side.

You probably want to hearth to get out, but if you need to get back out on foot, try to be at or near full health first. If you're lucky this will only take about 60% of your max health. If you have high stacks of Caustic Fog and/or can't get out of the slime fast enough, this CAN kill you. Swim across to the rocks by the slime-waterfall at 65, 84.15. The best spot to jump back up is the inward notch in the first rock. Jump across the waterfall to the shore; mount up first for a higher chance at not touching the slime. Follow the "path" back to the wall and jump up onto the paved ledge (the same one from option 2). Follow the ledge all the way around and go up the hill at the end.

I ran to the top of the hill behind the right side of the rear building /way 62, 82 or so, then used a Goblin Glider, and flew straight to the crate. Easy Peasy.

Just collected Kevin's Party Supplies from the chest at 72.1 88.8
My covenant is Night Fae, so as others have mentioned, it is uptainable for other covenants than Necrolords.

To get to this chest, I ran along the ooze just south-east of Scunner's platform, around the walls, and followed the mountain range all the way south and then to the east at the very bottom.
The chest is located inside the mouth of a tunnel with a river of slime oozing out of it, just at the edge of an oozefall.

Two things that help traversing the slime pools. Get Personal Air Filtration from Pandemis to negate the fog debuff. Also, underwater mounts work in the slime.

Necrolords only: inside the Seat of the Primus, take the portal to the Overlook (the roof). Jump off, use Goblin Glider Kit, glide to the chest.

To reach the one at about 70 75 I went South from the flight master, past Scunner, jumped with a glider from the wall at 63 79, flew to the island at 66 75, crossed over to the west, jumped down next to the waterfall to the stairs at 68 77 and from there you reach it easily.

This is rogue specific, but there's a very easy way to negate the Concentrated Plague debuff/damage from swimming in the slime. It appears that the debuff only applies when you initially enter the slime and ends when you exit, but if you avoid the getting the debuff when you first jump in, you won't take any damage regardless of how long you stay in the slime. This makes it much easier to get to the crate.

What's this mean? Use Cloak of Shadows right before you jump in the slime, and you can swim around in there indefinitely without taking damage, as long as you don't leave. This only works if you cloak right before jumping in; once you're already in the slime and have the debuff, cloaking will get rid of it, but it will come back once cloak wears off. But if you cloak before getting it in the first place, it won't start.

Not sure if this would work with other classes abilities.

If you're aligned with Venthyr: While the chest was on the south west area (co-ords from above post 65.9 / 88.4) I jumped down from the wall, jumped into the slime and used the Venthyr covenant teleport ability Door of Shadows to port to the island.

You can ride Iron Phillip in the ooze and not take damage. You just have to make sure his head doesn't go under.

I'm trying to nail down the conditions for when these spawn. One always seems to be up during the 3-day window coinciding with the "A Call to Maldraxxus" calling - the one where you fill your bar to 100%. Its availability does not seem to be affected at all by which covenant your toon is in, or whether your toon has finished the calling or not. You can loot the crate 1x a day per toon.

I've also seen the crate up at other times, like today - 1 day after the Aiding Maldraxxus calling is over - so there's another factor in play. The "Rare Resources" calling is up right now so perhaps that's it? For reference, the other callings the day of this post were "Aiding Bastion" and "A Call to Ardenweald".

Note: you cannot see this chest on your world map unless you are IN Maldraxxus (being inside The Seat of the Primus doesn't count).

The crate seems to have similar spawn rules to the Bleakwood Chest in Revendreth - always there for the 0-100% calling for the zone, and sometimes there when not (although having the Rare Resources calling up does not seem to be sufficient to spawn the Bleakwood Chest - it's not there today, so no clue what the other condition might be).

So, after having 35+ attempts and still having no pet or toy from the Slime-Coated Crate, I started to be desperated. Rarity was showing like 98 % unlucky on toy and 68 % on pet, I was farming it on 3 characters (Venthyr, Necrolord, Kyrian, with and without all quests completed in Maldraxxus), yet still no results. Usually, I just come to location and used Goblin Glider to get there quickly, but today I tried a different approach: I picked up plague mask, used teleport, and went all the way on my ground mount only. The toy finally dropped on the first attempt trying it this way!
Maybe it was just RNG, but I have a strong feeling that the glider was ruining it all along. Could anybody please confirm that he got the toy/pet while using glider?

Edit: Finally got the pet, after 93 attempts (only with flying to chest). Thank you all for answers!

All Shadowlands Chests appear to be a random daily reset that switch from zone to zone. Had Maldraxxus chests 3 days in a row, then Bastion.

I am so confused about these things. Do they still exist? I've gotten the coordinates from here, and flown around on multiple days, looking for them. Not just I don't get the pet, I never see one to open. Today I made sure to do it on a day when Rare Resources was up as a calling, and still, not a one.

Caution!

Since 9.2.5 had hit live servers, the sub-zone House of Plagues has its own chat channel and also phases out the crate until you're actually there (similar to the rest of SL zones for their own treasures).

Thanks to Remte and Maizou for sharing this info over discord servers of Warcraft Pets and WoW Secret Finding & Collections. The latter also made a bug report in the NA forums.

The 9.2.5 new storyline "Return to Lordaeron" includes a phasing of the House of Plagues to face the Lordaeron Blight at around 68 85.

If you are on this quest chain, even where you do not actively have a related quest in your log, you may phase and be unable to collect the treasure. This occured to me for the treasure located at the base of the tower approx 68 86.
