I used this item just outside of Mezzamere, Nazjatar while swimming in deep waters and was teleported to just north of the entrance to Freehold dungeon. 1 hour cooldown
Second use sent me to Drustvar north of Anyport. This item looks to be a random teleport if you find yourself in deep water.

I'd imagine this item will be just like the one from Warlords of Draenor or the engineering wormholes, where it teleports you to seemingly random locations, however when the stars align, will teleport you to a special area for a bonus treasure or encounter.

I kinda was hoping that this would send you to an island expedition zone without the whole "race for Azerite" gameplay, where you'd be able to just explore and stuff. I guess I was expecting for far too much. ^^'

I thought this was a toy I immediately "used" when I bought it in Nazjatar. I got sent to zuldazar. Thankfully there was an assault going on and I landed in the middle of one of the WQ.

I've used this item extensively to test it, obviously as per the other comments it works anywhere in Nazjatar as the whole zone seems to count as deep-water.

When using it from Nazj you end up in random locations around Kul'tiras or Zuldazar that may or may not be helpful. However if you use it in deep water when not in Nazjatar you are teleported TO Nazjatar from anywhere. The issue with this is not every fatigue water is counted as simply 'deep-water', the criteria seems to need to be in Kul'tiras and Zuldazar as well as I haven't managed to find anywhere else that it works in, but this could just be where I have tried (Currently Eastern Kingdoms, Northrend and Draenor).

The absolute best thing about using it outside of Nazjatar is you are actually picked up by a whale shark! If you haven't done so I would recommend it :D

I've ended up in a few random puddles inside Nazjatar as well as the pool of water near the berry-smashing Giant when relocated so there is more than one transport location also. ^_^

This has to be intended for use when getting Baa'l. At the final pebble, way out in the fatigue zone. I wish I'd had it instead of all the shenanigans and deaths I went through to get out of there.

Kinda dumb that this isn't a toy and takes up bag space. I used it next to the Unshackled quartermaster, thinking I was adding it to my toy collection then ended up in Nazmir.

As of 8.2.5 this item now does nothing.
No patch notes or anything related to it.
It can be sold for 125 gold though.

This has been fixed in a hotfix

Tried to go east from Boralus, reached Great Sea, near the fatigue margin.
I knew that being long time under water can lead to suffocation of my characater, so I've put on Band of Icy Depths beforehand.
Then I clicked on Conch of Wa'mundi. It worked, and sent me to... nowhere. I was immediately disconnected.
The character choice screen showed me as dead (ghost).
Any attempt to reconnect caused immediate DC again, without any possibility to press any button.
Disabling addons did not help much, except that now I could enter the game faster, and could fast click the resurrection button which made me dead again immediately.
Tried this sequence several times, with the same result: DC each time.
So I had nothing to do but to choose the on-site "Unstuck" service, and it helped: moved my ghost to the nearest graveyard.
From there I tried to go to the death place, accepted resurrection and immediately found myself dead again in the same graveyard.
Then I started to move to the death place slowly, by small steps.
At start, when I came close and got the resurrection button, its clicking did nothing. You click, and nothing happens. You click several times, and nothing happens.
Then I moved closer to the death place, both by horizontal and vertical axes, trying to avoid coming close to the fatigue margin - until the button did something.
Finally, I made myself resurrected at that death place, and flew away to repair from those many deaths which I had.
Conclusion: use Conch of Wa'mundi carefully, you may get yourself stuck, dead and DC-ed, and this comes in a loop.
