I have a similar macro, but I don't announce whom I'm resurrecting. Mine currently looks like this:
(If holding Alt (any form), cast Lifeblood;
in caster form, cast Rebirth (in combat) or Revive;
in any other form, cast Lifeblood.)
#showtooltip
/cast Lifeblood; Revive; Rebirth
If you're not an herbalist, this would boil down to:
#showtooltip
/cast Revive; Rebirth
And like Psychoholicx has, you could put at the end
/s I am resurrecting %t
The only downside to this is that the announcement will be made even if the spell isn't successful... like if you're in combat and Rebirth is on cooldown, or you don't have enough mana to cast the spell. Also, it'll announce before you start casting if you don't have the person targeted (in fact, I don't know what it will say if you don't have someone targeted because %t will be empty). But if you want to include the announcement automatically, I don't know that there's a way around this.
Alternatively, you could use:
#showtooltip
/cast Revive; Rebirth
/stopmacro
/s I am resurrecting %t
In this case, you could hold down Alt when using this macro if you want to make the announcement (i.e., you're sure it will work, and you already have the dead person targeted); if you don't want the announcement to go out, don't hold Alt when using the macro.
I'm probably overthinking this as usual, but there you go.
