If i'm not def capped, i use def rating in yellow socket.
If i am def capped but not hit capped, i use hit rating in yellow sockets
As of yet i havnt been both def capped and hit capped without gemming for either. Though if it ever happened id try to put a 41stam gem in the yellow socket if i didnt have 3x red sockets with them already with it.
I use stam gems in blue sockets.
I use the JC only 41stam gems in red sockets.
Assuming def capped:
22agi cloak enchant
275 health chest enchant
40 stam bracer enchant
parry/threat gen glove enchant
belt buckle with stam gem
22 stam boot enchant
If i wasn't def capped id put the def rating enchant on the cloak and chest. I'd enchant these 2 pieces before putting def rating gems in yellow sockets.
Helm/Shoulder/Leg enchants are fairly obvious, which is why i didnt mention them.

Best red gems are 16 parry or 16 dodge.
But always try to avoid socketing parry, it gives less avoidance than dodge and due to forceful deflection we hit DR with parry rating easily.
Not to mention that you could always socket +8 dodge/expertise +12stam aswell.

I agree with this suggestion. My priorities as a tank:
540 Defense
8% Hit
40% avoidance
Stamina
Stamina
Stamina
Stacking avoidance like crazy gets you into a bad situation for a few reasons:
Diminishing returns: your rating increase counts for less as these stats get higher.
Avoidance failure: Avoidance
will
fail at some point. You'll get a string of hits which results in what looks like dramatic health loss if you haven't been trying to increase your health pool.
When I meet the first three criteria, I make stamina the priority. I will throw some more stats at dodge or expertise by gemming with a
Guardian's Twilight Opal
or
Regal Forest Emerald
when I am trying to meet a red socket requirement. I'm not too concerned about hitting the expertise cap as either melee or DPS, but I do stack it when my other priorities are pretty solid.

I'd also put 6.5% expertise under the 8% hit and above the 40% avoidance if your blood or frost spec.
Also, 40% is pretty low. I'd say 50% should be the minimum. DK's in our guild are avoidance tanks. If they want a mitigation tank they have a pally or warrior do it, if they want an avoidance tank they look to druids and DK's. So i have to compete with bear tanks having over 60% avoidance.
