I have Swiftmend, LB, and rejuv bound to my mouse for on the run healing (wheel click and 2 thumbs buttons), then:
1-regrowth
2- WG
3- nourish
4- NS+HT
and that takes care of every heal in my arsenal, all within reach of WASD for movement.

yea thats the NS+HT macro. It will say cant cast while move, but just mash it a couple times and it will cast.

I base all my hotkeys on the buttons surrounding WSAD then I use grid along with mouse over macros to heal.
In treeform I use a mouseover macro for each healing spell (heal on mouseover, if none then target, if no target then me) and in caster form I have all of my spells on a/cast spell; healing spell macro which basically heals a friendly target hurts a baddie.
Might share the macro's (they're around just not as filled out as they could be) if I could go online atm...
Keys:
q-e-r-f-g-c-v-x-z-shift+F-1-2-3 for all my spells (level 80 much?) and that pretty much covers my entire spellbook.
shift+Q-shift+E-shift+R-shift+C-shift+V-shift+Z are all bound to different forms. Oh and shift-G to break snares.
I don't have anything unbound so I don't get key clutter, 1 actionbar only visible and it's good crud like my hearth trade skills and such on it. I mix it up between pvp, healing and dual-specs which is why it's so complicated.
