You can leave notes on your friends in the default UI.
Open your friends list, click the little sheet of paper icon next to their name.
Don't worry, I only discovered this a few months ago, and I've been playing for years. :)

is another one. Like Karma, it allows notes to be taken on any player, not just those on your friends list.

The fun thing is. This was added with the 3.0 patch, so it haven't been here for long.
