I've cured my problem with duel tweekers with AutoDecline. Very nice addon. I still run into schmucks who think by spamming duel requests they can somehow overload my addon or otherwise inconvenience me.

People assuming that because your class has a healing tree - you are a healer.
(Same could be said for tanks I s'pose, just never have that problem since my warrior IS a tank)
I feel the opposite. I get irritated when people assume that because they don't have any points in their healing/tanking tree that they *can't*. My 59 Fury Warrior might not be the best tank available (since the lack of a shield nerfs defensive stance and armor) but I can still tank without many problems.
If your class has healing abilities, why are you *not* leveling them through your trainer? More abilities means more usefulness and flexibility, especially in emergencies.

People assuming you do a job because of your class (warrior = tanks, druid/pally/shaman/priest = heals)
People needing items for multi-jobs when someone there can use it (like ret pallys needing healing gear "just in case" when the healer that IS healing needs it NOW, personal experience),
elitests,
drama,
beggers,
lvl 1 orcs that invite you,
when you get a group for something everyone goes "can I get a summon"... frikin lazy.
when the group wipes and they wait for the healer to run back and res,
people who over-complicate the game by with the "this is better then this because ...",
people who take a game too seriously,
people who dont use the right chat channels (general chat is NOT the guildrecruitment channel, trade is not a LFG or general chat...),
people who whine about not getting the gear they want...,
70's who camp lowbies (seriously pathetic, just kill and be on your way.. but do u have to camp them!)
botters,
The GM's on wow (not all of them, just most of them),
people who don't repair before a run / raid and don't come prepaired,
people who DONT use bandages in an instance (looking at you rouges...),
people who don't thank the tanks or healers to dedicate there toons to just that,
people who overprice crap in the AH,
people who jack up prices when something comes along (like now that winters veil is here, eggs are like a gold a peice... THEY DROP OF NEWBIE LAND NEUTRAL MOBS!)

Taurens who must mount up (if aplicable) and stand on a NPC... (MOVE IT FAT ASS)
Gnomes, and there uselessness as footballs (everytime I go to punt one it seems to backstab me...)
Kids who play WoW (esp. the teenagers!)
People who ask stupid questions in game,
People who don't use websites like WoWhead or thottbot to look stuff up,
People who ask for advice, you give it to them, they ignore it and do what they want instead then wonder why it all went to hell...
God I don't think I have the time to list it all. But generally speaking everything the general populas does annoy's me. So yeah, I typicly don't have any chat on other then LFG when I want to go, or guild.
(But it should be noted, that the general human populas annoys me too... sooo.. yeah I'm gonna grow up to be the old guy that yells at people for standing on the grass...)

One I think no one's mentioned yet ....... it annoys me when people leave a guild and give absolutely no clue as to the reason. As a guild leader, it's helpful to me to know where we need to improve, but if no one says anything ..... well, yeah. -.-
