I have dual spec fury/prot on my warrior. Or, more accurately, I
did
have fury/prot on my warrior...until I swapped the prot OS for arms PvP OS...and then signed as a tank for LFD. Probably would have been ok if we hadn't got PoS :(
It occurred to me after I sheepishly apologised and offered to drop group (everyone had a good lol and the retadin offered to tank so long as we were ok with his mostly-blue gear) that I never even signed as a tank for LFD when I
had
a tank spec!

In ICC, I was trying to pop a potion. Spun my mouse wheel too quickly, and ended up on another caster bar. Activated Bloodlust instead. On trash.
It's not like I could hide it, with everyone growing larger, and the "MuuuuRAWRRAAAARGH!" sound.
"Um, Oni?"
I now keep Bloodlust on a separate bar, at the bottom of my screen, where it must be clicked on.

I've Bloodlusted trying to click my buff food before a boss, once.
And on my tank, well -- let's say learning how to tank is a curve. I've !@#$ed up my fair share of times -- the latest I can remember being misclicking Challenging Shout on Sarth 2D when we were about to gib him (back when it was a big deal, I was tanking Sarth) -- but the best fails are the ones you make up for in the same fight.

Trash before General Vezax. I'm still a trialist in my first serious raiding guild. Something goes wrong and we wipe. Soon after, most of the raiders are waiting for one slow guy to run back so we can buff up and go.
Waiting patiently, I lean forward to grab something on my desk and accidentally press Blink. My mage ends up right in the middle of 2 trash packs, dying instantly. With no one prepared, my guild tries to save the day, but fails. My Blink killed 24 people.
I'm still in the same guild to this day, but when I think about this, I can't help but feel embarassed. I'm glad they laughed it off instead of complaining.

So back when icc first came out my guild was doing our first attempt on Dreamwalker. Well needless to say we wiped, i survived because i was in the portal. So i run out the door and rez 2 people. Well i had forgot to pull up my totems and when the boss spawned i was shut out and the 2 people i just rezzed died again. We were all laughing and i rezzed them again. Well........i forgot to pull up my totems AGAIN. Nobody else seemed to notice either and both of them died for a second time. They were not mad or anything and we all had a pretty good laugh.
Edit: Thought of one more fail that goes along with the poping bloodlust on trash thing. I used to have heroism and my water shield binded to similer key strokes. Heroism was Shift-6 and water shield Shift-7. I had to move heroism after a wile because I kept accidentally clicking hero instead of my water shield while in the starting gates in 2v2 =(. My partner was not amused after the 5th time lol.

No, what sucks is the fact that the only time in his life that Blink actually worked properly, it put him right in the middle of a pack of mobs =P

this one just happend to my sister, on the quest Fate, up against your will. Instead of waiting 5 secs and taking the portal she RAN all the way to org. I told her that she shouldove takin the portal and she gets all mad and says I DIDNT KNOW THERE WAS A PORTAL! sad part is, the portal spawns right infront of you. >.<

When I first discovered keyboard turning in all its glory, it was a tank in MgT hc. one mob had gone lose and aggroed me as ranged, it was at maximum distance, and the tank managed to retreive aggro by running with "W" and "D" creating a cirle from where he stood to where I was, that took many seconds, and I was already dead when he came to aggro the mob.
clearly the biggest keyboard turning fail I've ever seen. It was pure. He faced the opposite direction from where the mob was, and it takes a while to get 20+yards by pressing W first and then D to finally get there

Forge of Souls, on the last boss, as a healer. Not sure what was happening, but the group was losing health left and right (I'm guessing that they were damaging themselves from the reflection buff on the boss). So dps was losing health. Tank was losing health. I was trying to save everyone, but I was saving no one. A dps died, followed soon after by tank. Thinking it's a wipe and not wanting to everyone to have to corpse run back, I postpone the inevitable and ran away and shadowmelded, so I can rez people after the wipe. I was seriously standing there for about a minute before I realized that the other two dps weren't dead yet. This confused me, but I thought "what the hell, let's see if we can't finish it". Came back and resumed healing. Somehow the three of us downed the boss.
Epic fail is me bailing out of the fight. I am still embarrased. Epic win is the dps. They weren't going down without a fight. I've seen the "last dps standing" routine enough times that I should have known better. That corpse run is a b--- though :)

I had a really epic fail today. Queued for random on my draenei DK, and got Slave Pens. When I zoned in, I immediately knew I was in for a wild ride.
First: The tank, healer, and one DPS were all from the same guild.
Second: The tank, who was a pally, was using a polearm.
Third: The hunter was suspiciously higher level than everyone else (he was 68, everyone else was 60-63).
Fourth: The pally had talents in all
three trees
and had not filled out a single tree to the 51-point talent.
So, immediately the other DPS, a gnome warlock, asks why he was using a 2-hander, then after the first pull (of course the tank couldn't keep aggro), he left. Also, the tank kept asking if he should be backup heals. Um, yeah, the tank should also heal, makes sense.
Then another warlock joins the group, and is from the same server as the others, so I'm thinking "oh great, another one".
But no, this person was freaking AWESOME. Seriously. She was so cool. I filled her in about what happened and we exchanged jokes and such about the other three. The tank died a couple of times, the healer died a couple of times... And half the time the hunter's pet was tanking.
Some other strange things... The healer was in full heirlooms, and was pretty good (although she wasn't using innervate and asked the warlock if they wanted it), and the hunter was wielding Hellreaver (yeah, because strength is good for hunters...).
Even though it was a fail group, we actually managed to make it the whole way without a wipe (not like Slave Pens is challenging) and it was pretty entertaining.
