I got quite a few more Fails, From some IRL friends who also play wow. Wall O text inc.
Fail:1
While in a ICC 10 man pug my Paladin friend was main tanking (or OT cant remember) with only the first 4 bosses up everything goes a smooth as butter, until "gasps" Death-bringer. So The first wipe was because he go far to much blood power not sure from what might have been people not spreading out? Any way raid leader explains tactics, everyone says yes. So they pull everything goes well until the 2nd summon of blood beasts, So the the first one gets kited well but the second one aggros the druid healer and for some reasons he just loses it and panics. Just stood there and spammed heal on him self while the blood beast was wailing on him. It gets worst, for some reason ALL melee dps start attacking the still up blood beast and as you might have guessed get aggro and proceed to die quickly. Now the raid leader goes ape @#!$ types in all caps in raid warning "MELEE ON THE @#$!ING BOSS!" So the warrior tank dies cause everyone is in panic and my friend quickly: leaves the raid, bubbles and hearths back to dalaran. Lessoned learned eh?

My druid friend was balance dps and went to 10 man TOC normal pug, Long story short, he dies from fire from snowbolds and joking in the raid chat he said,"ok wipe it guys" and they did ><
Funny thing my friend wasn't even raid leader XD

Back before Ulduar had come out, I was in a Naxx10 PuG with a friend. We had cleared to KT with no real problems, just the normal grind there. I was the MT and was just bored and worn out by the time we had engaged KT (the add killing phase didn't help that). While fighting, I kept zoning out and sort of mashing all my key binds as I was no longer 'looking' at the screen. I suddenly snap my attention back to the fight and notice a void zone under my feet...I died. The other tank, who was in dps gear for the fight picked it up while I got out of my chair and rolled around on my floor (I actually did roll around). While I was on my floor, I got a B-rez, which I ended up taking about 10 seconds after it was given and everyone was yelling at me in raid chat to take it. I took the rez and the fight resumed with no real noticeable damage. I zone out...again...I notice a void zone under my feet...again...I die...again. We wipe.
Second attempt -- I zone out, stand in a void zone, we wipe.
I forget how many attempts it took for them to have the other tank take KT. Ahh, the good old Naxx days.

I've got two: one of them is me, the other happened today.
1.) On my warrior(which I still play), I tanked instances in Arms up until 78. That was the first time that I had heard that Arms was not for tanking stuff. However, in that run, and many of the ones going all the way back through good old RFC, no-one had died. Funny...
2.) I was in H Occulus by virtue of the LFD system, and we had just downed Mage-Lord Urom with me and the other dps dieing due to frost damage. The healer then starts to rez us. After the first two rezzes, the chat log looked a bit like this:
DPS #1 has died.
DPS #2 has died.
DPS #1: WTF
DPS #3(me): LOL
Turns out the druid had rezzed them while standing in the frost damage zone.

I've seen DKs, hunters, warriors, paladins (DPS) gem for SP, Parry, Dodge, Agi (Str on hunters), defense, intellect, etc.
Gemming and enchanting armour are far too common.

Here's another one.
I was doing a ToC10 on my Shaman with a few guildees and some PuGs. One of the PuGs is a Hunter, who just so happens to be the fail of this story. Now, we were all just chatting on Vent and we told the PuGs to get in if they wanted or if they weren't completely sure what to do for some fight. The Hunter replies, "What's vent?" That set off a flag, but we didn't really care about it at the time.
Beasts -- First fire bomb on the Hunter, he dies. We wipe on that, which was the only real wipe during the entire run :P On the second attempt, he dies again somehow....I don't know how.
Jaraxxus -- I don't know what he's doing, but his DPS was very low...2k and below.
Faction Champions -- Wasn't paying attention to him.
Twin Valks -- This is one of the best parts. We get them down and we look at the DPS meters for fun. The Hunter's DPS was 1358 and his dmg done on the fight was miles below the tanks, I kid not. He was alive for most of the fight and died just at the end.
Anub -- After his death on Valks he states that he must repair and we wait to drop the floor until he leaves the instance. Once outside however, he asks, "Where's the closest repair?" One of our tanks goes out to show him. He comes back in after the floor has fallen and says he couldn't find it and his gun is broken, so he'll use his Polearm to melee Anub. Although, he butchered so many words in the sentence that contained that, that it took about 3 minutes to decipher it. We all laugh a little, and I state, "I don't want him to fail here, but I also want to see if he's seriously going to melee." But, we decided to just kick him out, which was a bad idea because he got very irate and pulled Anub :P
Anyhow, that's another one of my stories completely set in one type of sentence structure.

Call your self a "HardCore Raider" and have 12 or more Deaths in a single raid.
Pst. Hardcore means you play alot, not that you FAIL alot!
Call youself "Casual Raider" but spend 12 or more hours a week playing Wow.
Pst. Casual means you don't plat alot, not that you can't complete sinmple raids
Math fail.
12 hours a week comes out to about 1.7 hours a day, which sounds pretty casual to me. Couple of random dungeons and some farming before bed. Putting a couple of levels on an alt. Even if you claimed to be basing this on a 5-day week, that's 2.4 hours a day for 5 days, with two days off. Still pretty casual.

Oh I've seen a ton in my day, but I think my best was back at 70, on Hex Lord Malacrass on my resto druid - I needed mana so I popped innervate...when I somehow had the enhancement shaman targetted. Thus, he gets his spirit-based regen incereased (this was the old-school innervate, for those who remember) which amounts to virtually nothing.

I had a group today for Stocks. I was healer, warrior was tank, and shaman, kitty druid, and paladin were DPS.
The shaman didn't use any totems. At all.
Well, he used a Stoneskin totem for the last boss.
