My guild's pally tank continued to tank Gruul while he disconnected. Didn't lose aggro or anything.

Mine aren't exactly the good variety but I would say either
a) during a stalemate in wsg I was holding the flag on my pally. At the time my trinket was bound to an action key right next to my bubble. Warrior charges in and hamstrings me and guess which key I pressed on accident.... ><
b) being a dumbass before a Rag pull by PW:shielding myself (my old main was a priest) and jumping in and out of the lava. Then I accidentally jumped a little too far and pulled rag while people were still getting fire pots and buffs. weee!

Maiden of Virtue fight.
I was DPS...tank goes down with her at ~75%. I switch to bear form, change to my tanking weapon and we down her no problem. With the exception of my weapon I was in full DPS gear o_o
The raid leader kept calling for a wipe, but everyone kept at it when they saw I wasn't getting spatted.

Had two PvE "Holy ****!" moments, both in Outland.
First was fighting trash in Hellfire Peninsula when I noticed the ground shaking. Turned and aimed the camera up just in time to see the
Fel Reaver
and hear it's metallic shout. Literally jumped in my chair.
The second was virtually the same thing when I turned and saw
Durn the Hungerer
in Nagrand.
Ah, same here man. I have NO idea how something so frickin' huge can sneak up on you, but it just happens.
The Devilsaurs in Un'Goro do the same damn thing !
But thats ok because I always go back and own them on whatever toon I just brought through there.

The same thing happened to me last night. The MT died to I popped into bear form and took her down. Luckily we have some really good healers.

Bad one was pulling maiden by accident, while not everyone was in the room, and half were drinking. Or possibly the time our MT on Maggy said "guys, hate to tell you but i'm wearing my DPS gear" partway through :P

Another holy ^&*! moment for me was the first time I chain killed 5 70's, a 1v3, then two more in close succession afterwards. I thought to myself "Did I really just pull that off? $%^& yeah!" and that was followed by "!@#$ NO FRAPS -.-"

Oh man, a great moment in the history of my "wow career". Blackwing Lair run, Nefarion, and a wipe, I had the sense to "PRINT SCREEN" as Nef was about to 1/2 shot me (mage class), the screenshot showed me as a tiny portion of the bottom of the screen, and Nefarion as the other 200 percent : )
That was my desktop background for about 6 months after that. Huge Dragon, all of the screen, down below is me. My brother sees the desktop and says, you guessed it..."Holy SH@#"

in the night elf water dungeon instance that i cant renember the name of, i accidently turned on all candles on all at once. my lv70 mage i payed 10g to boost me through died. so did i. now hes corpse camping me with his horde alt untill i can send him an EXTRA 10g although it costs less than 2g to repair, he says its my fault for not healing him. even though he constantly ann oyed me about how my heals are insignificant to his greatness. this is my holy **** moment.
Hahaha... what a horrible mage. Couldn't take on a bunch of low mobs? Lol fail. I wouldn't pay... in fact if I was you I'd be demanding a refund for being just plain bad.

Good holy *!@# moment, we did bwl at lvl 70 and the mobs before firemaw have weaknesses that change from this to that. I got an 8k envenom crit on one of them when he was weak to nature. Me on vent: "HOLY ^&*!"
