Is Colossus Smash Bugged?

Since Mists launched I've noticed that my colossus smashes are missing
a lot
. When I was levelling I just assumed that it was because my stats were all over the place, but it's still an issue at 90.
I'm not completely hit and expertise capped yet, but close enough that I shouldn't be missing at all on anything less than a raid boss (about 0.5% chance to miss and 1.5% chance to be dodged on a level 93 target), yet I frequently see CS missing three or more time in a row. The peculiar thing is that it still applies the sunder effect, from the glyph, but doesn't cause damage, apply the CS effect or trigger the cooldown. I have yet to notice any of my other specials missing or being dodged.
I've googled for any mention of a bug or anyone else experiencing this, but have drawn a blank. Is it just me?

As I say, I'm close enough to being capped that missing/being dodged several times in a row is a near impossibility. Likewise, I have never noticed any of my other specials missing. I also experience this on level 90 mobs, which I am hit and expertise capped on.

What is hit cap? In your mind? What I mean to say is what do you think it is? I'll test it today when I get on.

I go by the percentages for both (7.5% each). I'm sitting at 7.14% for hit and 6.14% for expertise. In other words, 0.36% chance to miss against a boss level mob and 1.36 chance to be dodged.

ok fine. Gear has not given % since vanilla... When you mouse over your % it tells you the numeric rating.... Which is how the rest of the world knows what number to reforge in and out of...

The character pane shows percentages, though (as well as rating) and also shows percentage chance to hit against mobs of different levels. That's what I use when reforging. I thought everybody did.
