I just finished the survey. Your questions are leading and telegraph your implicit bias. Shame on you. Your research will not yield what you think it will based on the purpose of the study.
Also, asking questions about behavior in such a way: "I have never gotten even with someone." (not a question you asked but representative of your questions) forces me to answer "False" because I'm CERTAIN that in game or out, I have done that. (I can remember incidents of that when I was a child with my brother.) So I have to answer that I have done, but the tone of the questions was looking to see if I was petulant

And leading questions are worthless.
i have too agree with this. some of the questions we're really biased and had only 1 possible answer unless you were a saint that never did anything bad since your birth (which is impossible).

I can agree with felhunter and Halakah. Many of the questions had only 1 real answer unless you have lived in a wooden box for your entire life and never spoken to anyone or done anything ever.
Most of the questions were "Have you done X" with the possible answers being No, Yes a bit, Yes quite a bit, Yes a lot, Yes loads. The answers that you get, PossL, are not going to show you whatever you're looking for. To take one example - every single person in the game has thought about WoW out of the game at some point in their life, so they are forced to answer "Yes, I sometimes think about WoW outside of playing it" - this does NOT mean that everyone is always thinking about it.
Make some proper, unbiased, good questions if you want to get relevant data.

Upon closer inspection I agree with the criticisms given by users. I'll endeavour to be more thorough in my examination during the approval process to prevent similar surveys getting on to the forum.
