I written a similar post like Atik not too long ago. Seeing 2 gays in public holding hands is equivalent to a smoker standing next to me and pulling out a cigarette. I'm not saying it's wrong to be gay, but I feel disturbed when they are near me, although they were born from 2 heterosexuals. :(

Your analogy fails because in the former, there are actual possible negative health consequences (i.e., second-hand smoke can lead to cancer, emphysema, etc.), while in the latter
you
are inflicting the problem on yourself.
In other words, your reaction to two men holding hands is your problem, not mind; if you don't like it, don't look, but don't expect me to not hold hands because it makes you victimize yourself.
Your irrational fear is just that - irrational. Feel free to get over it, but don't expect me to change anything I do just to make you feel better about your irrationality.
I support gay people, but the questionnaire just doesn't make sense to me. The one who made this seems to purposely skip everything bad about being homosexual (read: not being able to have children and stuff like that)
Homosexuals are not infertile. A gay man can father numerous children without ever getting near a vagina; a lesbian woman can give birth to a child every nine months if she wants, without ever seeing a penis.
I didn't read the questionnaire, but I wanted to toss out that correction there. Homosexuals will never produce a child through direct copulation with their partner, but they can reproduce through other means without any real difficulty.

Yes, true. I just always feel the need to point out that being gay doesn't make you infertile.
There are a number of anti-gay activists who try to propose that legalization of same-sex marriage would lead to the extinction of the species, which is just a stupid statement; first, it presumes that doing so would lead to
all
heterosexuals becoming homosexual, and second, that homosexuals can't reproduce - both of which are false statements. :)

While sad....there is the update at the bottom there:
UPDATE: Alfarena Ballew from the Marion County Coroner's office called to offer this statement, "Her friend and her aunt are working together with the life partner to take care of the remains. We have nothing in writing from the partner asking to claim the body. Our records show that the next of kin is her aunt. Our understanding now is that they're all working together to release the body and take care of the services." Ms. Ballew described the incident as a "misunderstanding" and says the office is on track to release the body shortly.If the Aunt was listed as next of kin, rather then her partner, it would make sense they wouldn't want to release it directly to the partner.

Perhaps this shed's a little more light on the entire subject?
The first quote is your feelings on intolerance, the second are my feelings on homosexuality. While you see intolerance as irrational, I see homosexuality as irrational. Further, there is little chance of either side changing thier opinion.

No, not really.
A phobia is an irrational fear of something - i.e., a fear that is not based in logic or in the reality of the situation. Irrationality is an innate part of the suffix's meaning. By extension, homophobia is innately irrational.
On the other hand, sexual orientation has a grounding in reality; sexuality serves a biological purpose, even without the act of procreation involved. While not conclusive, there are numerous studies (stretching all the way back to the 1970s) indicating that sexuality, while fluid in some individuals and immutable in others, is a biological trait.
If we really want to simplify it, look at it this way: I can provide reality-based justification for my sexuality in numerous ways; can you, or anyone in the "it makes me queasy to be around homosexuals" group, do the same? So far, that justification has amounted to, "I just don't like it," - not a really solid, rational basis for the reaction.
