I have 3 that are related.
Back before the nerf, I was in a 10 minute fight with the first boss in sfk because neither the tank, nor the DK was interrupting.
And just last night on an alt in normal VP. Got a pull with two adepts (healers). I was healing, and I watched as all 3 players with interrupts (rogue, tank, dk) never interrupted any heals and in fact were focusing on the non.healing mobs. OOM wipe
Finally, in the same dungeon, where you have to pull the dragon out of the healing well. The tank didn´t do it. and when I was telling him that he needed to pull the dragon away from the well.. he said something about ´shut up, don´t make me rush, I will go at my own pace´... OOM wipe
What is amazing is how oblivious they were to the fact that their targets health weren´t going down.

Let's see... worst heroic...
I was second dps all the way through H sfk and only one that interupted on first boss. I was the healer and top dps was tank... Can you say a 3 hour run? I wasn't going to abandon them in their pathetic state. Damn, I'm too nice...

Grim Batol run where the healer couldn't keep a raid geared tank alive, wouldn't dispel anything and swore at anyone that questioned her.
And she was in devo aura, stacking with the tank and refused to change it /facepalm

Me, when drunk, in heroic Stonecore. I was on my warlock at the time.
It was at the trash packs before Ozruk. My dumb-arse drunk self said "
oh crap, those mobs are going to go after the healer if I don't do something!
" and promptly flung a chaos bolt into the middle of the pack.
It's a fair reason, wanting to save the healer. Well, if you ignore the fact that the mobs were nowhere near us at the time, nor in combat, and thus were able to be completely ignored by our group on the way to Ozruk.
When I woke up the next morning and logged on, my damage metre still had the data from the dungeon. I was second overall, and alternating first/second on boss fights. I don't want to know how poor the other DPSers must have been if I beat them in that state (although my ninja-pulling probably helped towards the overall damage).
The tank and healer were friends of mine and not random puggers, if that somehow redeems me!
I also bought an
Impetuous Query
(on a warlock) because I liked the spinning animation... then they went ahead and "fixed" it. Cheers, Blizzard.
I don't drink and WoW any more.

Let's see...
Thinking your stealthed when moving in for a sap when in fact you're not. Always worth a laugh.
Worst one ever I believe was back in BC on my pally before ret was considered a dps spec by the general populous. Heroic Sethekk Halls, got that darn shrinking curse put on me twice, then got mindcontrolled by the totem, nuked our healer, and no one stopped to kill the totem.

Time for a confession, I guess.
Yesterday, I finally managed to get the iLevel of my Elemental Shaman to 330, therefore qualifying him for Cata Heroic. It's my first time ever doing C-H, and after half an hour, I find myself in Grim Batol.
Hearing about people ranting - no,
demanding
that DPS do at least 12K, I feel extremely self-conscious when, after all said and done, I only managed to chalk up 7.5K (mostly because I kept running away from one thing or another). A Hunter was kicked because his DPS is 5K, and the Healer too when he DCed - I think I'm spared because since we already kicked two people, it's impossible for the third one to be given the boot.
And oh boy, we must've wiped 7 or 8 times throughout the entire dungeon - my gear "health" went from 100% to 24% I think we spent around 2 hours doing this dungeon, although a small happy ending to this is the fact that we finally completed it.
So yeah, my performance in my first Heroic is not as atrocious as you would read in the other posts, but still it's not something I'm proud of. If you took so much time to complete it that you can actually Reincarnate
twice
... ;_;

I watched a guy wand a whole fight. Everyone else seemed too busy to notice but i was silently lolling, til he won the next roll >.>
then the reaid erupted into chaos aftersomeone posted the dps meter and he'd been doing like 500dps.

I got into Heroic Grim Batol the otherday as the tank.
The first pull feels like it takes really long, since I can pop Shiels Wall three times.
The healer posts the meters, I did 7k and the three DPS did 1k each... he leaves, I follow.
Seriously, how is 1k DPS even possible? I can do better than that auto-attacking!

My personal worst: Trying to DPS with a fishing-pole. Oh, and so far I've survived the Mario-phase on the Vanessa VanCleef gauntlet only once :X
Worst I've seen so far was Foe Reaper 5000. The tank, the healer and the other DPS (not in the prototype) manage to die 3 seconds into the fight... So I put Growl back on my pet, use Mend Pet every now and then, walk away on the harvests and overdrives and 'solo' the fight (not really ofc, as there was also someone in the prototype).
Hunters going melee is semi-easily explained, Blizzard likes to break my gun mid-fight... After the first 2 times that happened I got myself a second gun. Assaad drops a lot of them :)

Were they all mage/warlock/priest spamming their shoot ability from their wand? Only thing I can think of tbh

Personal favourite is SC (this fail wasn't even on heroic but hey you get that).
So there I am dpsing away on my warrior doing a slack 7k (lv 83 at the time) and get up to ozruk...
So doing the usual thing we clear the big ogre and I'm just lining up to take the pack of 4 just before him as well when the tank charges in and starts ozruk. At this point I begin thinking... what the hell is this tank on but whatever i'll go with it.
Sure as eggs the tank sits against the wall basically afking the fight since in normal you can just soak the hits. Then all of a sudden I cock up one shatter and go coasting across the air and aggro the pack of 4 mobs (yep bet you didn't see that one coming too /end sarcasm). Sure enough leads to a wipe.
So re-enter and assemble infront of ozruk and the tank starts going off his head "Who pulled that pack, I hate dying to stupid things... blah blah blah /rant"
I kindly replied to him that the wipe was his fault for not clearing the trash pack before him and that a simple cocked up shatter by myself lead to the pack getting pulled.
Tank attempts to kick me (total damage done 40.3% (overall)) healer denies the kick so the tank chucks a tantrum saying they refuse to clear the pack and I should have been standing infront of the boss soaking a ton of pointless damage.
Needless to say I cracked it with him told him to go stick it up his, chucked on my sheild and sword, changed to prot spec and promptly sent out a vote kick with the reason "jackass" which passed before I believe the tank even finished reading that I told him to go stick it.
We merrily continue on and after pulling the trash pack one of the dps turned around and said "#$%^ that pack took so long I think my beard grew back, I better go get my razor".
The rest of the dungeon continued without a hitch.
P.s. Yes this was a fail on my behalf with shatter but I figured I would lay out the whole context as I thought it was a good laugh and I also like to remind people that you don't have to be a tank to have authority in a group (actually in the last 2 weeks I have kicked around 8 crap tanks and yet i've maybe only kicked 1 dps ever as a tank who was underperforming (not counting those who go offline for more then 3 packs of trash)). For reference
my warrior
