Yes it's possible, and yes you still need exalted. Highly doubt it's possible to get exalted before level 20 though, as the runecloth hand-ins don't open up until 40-something I believe.

Just from doing all the quests in all the starting zones, 10-20 zones, and most of the 20-30 zones will get you to around 35+ and you can get exalted with all five cities easily.

Yes the you have to get exalted with a city before getting the mount weather or not possible to get exalted before 20 idk, but I'm pretty sure there's a way.

I worked hard on my Night Elf to get exalted with Stormwind before the first mount was available to me. I did my best not to get any XP unless it was connected with completing a quest, and I limited the quests I did to those that gave SW rep (as much as is possible). I was able to achieve my goal - but just barely. I became exalted with Stormwind less than half a level before I was allowed to train for the first level of riding. This was back in the days when you had to be level 40.
Based on that experience, and the fact that leveling is even faster now than it was back then, I'd say it's not possible to become exalted with any city before hitting level 20.

Ok, I'll reword my statement to be more accurate:
I'd say that it's not possible to become exalted with any city before doing enough work to have become level 20.
