If it was inactive theres nearly no way they could have jacked it, theyd need ALL of your personal imformation.
Incorrect... Its a fact that hackers pay for a month, reactivate an account, just to use it as an income. I have seen many complains about that on the Official customer service forum...
There is no way to find out unless you call Accounts and Billing. They can help you figure it out, and fix any issue. Otherwise if you just have achievements, take it as a bonus. You may be just forgetting what you have done... it happens...

You could have gotten a key logger and simply logging into account management can be tracked, the game isn't required. Sounds like you took some good steps to protect your computer, however no scanner can detect every bit of malware; if you picked up something obscure it may not be caught.
You can add an Authenticator to your account, this will increase security greatly. You can

for a few dollars or you can get a
