Using the 4th and 5th mouse buttons with a mouseover macro

So my problem is that I want to be able to bind cleanse/dispel/etc to my two mouse buttons triggered by my thumb. I use mouse-over macros with X-Perl to do all my healing, and I would like to have the same functionality with dispels/cures.
The problem is that the mouse-over macros just won't work using the buttons on the mouse. The mouse-over macros are just on my - and = buttons on my bar 1, and I have the mouse buttons bound to those keys. Oddly, the mouse-over works correctly if I mouse-over an actual player avatar, but not on raid or party frames. If I actually reach over and hit the - or = keys it will work correctly.
Is it a limitation of using the extra mouse buttons, or a problem with the way X-Perl handles frames? I can't bind the dispel directly to the mouse buttons because I would not have mouse-over functionality.
I hope that makes sense, and I am open to other addons (I used clique long ago but am not sure if I want to set it all up again just for dispels) or other ways of doing the macros if that will get it working.

That would work fine for the actual spells, but I need them to be mouse-over. I actually got used to using the mouse buttons originally when I first got the mouse by binding them to those two keys and then putting skills like kick/mind freeze and taunt on those two buttons. That kind of skill usage works just fine I am guessing because I am just using the skill and not a macro.
Does anyone know of a way to bind a macro to a button? That would accomplish the same thing without having to go through the step of binding the mouse buttons to a bar location and then placing a mouse-over macro in that bar location. It would also free up some bar space which would be nice.

I don't think you guys are understanding me.
I have the default Blizzard button bars. I have a macro for Remove Curse on the - button on the bottom bar. That button has two keybinds, the - button and mouse button 4. The macro is as follows:
#showtooltip Remove Curse
/cast Remove Curse
It is a super simple macro. @mouseover and target=mouseover do the same thing so it is not that.
The problem is, if I just hit mousebutton 4 while not targeting anything or mousing over anything it tries to cast the macro but fails because I have no mouseover target. If I mouseover a player/npc avatar it casts just fine. If I mouseover a unit frame like X-Perl or the default Blizzard frames it doesn't even try to cast the macro anymore.
I do not think it is Xperl anymore because I tried with default frames and get the same behavior. I am curious if this is just the way it is or if there is a setting somewhere I need to enable/disable to fix this.

You canse use @mouseover with mouse button because unit frames, like Elviswind said, eat them.
You have to:
- Use an addon like Clique or Dominos (not sure of spelling)
- Bind your mouse button to another key (like F6, F7 or numberbad) outside of WoW and then set your macro to these new key and the mouse will proc them
- If you have a Logitec mouse, you can use their Set Point Software (you go to their website and download it for your mouse). I dont know if other mouse have similar stuff.

That is very informative. I hate to setup clique just to use the dispel buttons, but I think that is a better solution than binding the buttons to keys outside of WoW since I also use those buttons on melee classes.

got same problem with my mouseover macro for dk's combat ress. while binding it to any key ("E" for example) it works perfectly. While i place the spell on a bar which is bound to middle mouse button - it works as long as i dont mouseover any unit or raid frames. shame, i had to replace those too spell with one another to get it working (over spell is consumption for blood dk)

The problem is still topical in 2021. To use my paladin healing ability Word Of Glory in mouseover with the "Next" mouse button (Button 5), I had to use Clique.
@mouseover in a macro didn't help, even without any addon loaded to avoid weird interactions. Normal buttons like "1" on the keyboard would work fine, but not the "Next" "Previous" mouse buttons.

Thank you very much for this topic, I was trying to figure out how to make my mouseover Beacon of Virtue macro work on Button 4 and thanks to the comments in this topic I finally solved that by using Clique
