Always roll with SotM. If the healer can't keep you up, that's their fault, your job as a DPS is to output the HIGHEST DPS at all times possible. Yes, staying alive lets you do more dmg, but if you're gimping yourself by rolling with FR gear on Sapph, or not having the correct seal up, simply because your healers are fail, its time to find new healers.

The only way I see us dying on Gluth to SotM is due to dpsing after he decimates.

A few weeks ago, I was tanking The Nexus HC, and all was going well until we hit Telestra.
Just some side information, I'm a def-capped Prot Warrior who has succesfully tanked several heroics but haven't started raiding yet (I'm waiting till my guild is ready to do some 10-man raids), and who has a great little add on (SkullMe) that marks my target with the skull, so it's obvious who I'm hitting.
Back to the story. We get to Telestra without too much trouble and we decide what to do. We agree on the order Frost, Arcane, Fire to down Telestra and I charge in. She starts throwing us around and then splits up. I charge in, thunderclap and focus on the Frost version. In the corner of my eye I see the Fire version attack the DPS warrior (let's call him Donald, I'm sure some of you will know why :P), so I taunt her. I continue tanking the Frost version (didn't need to pull the Arcane on me, since she was still focussing on me) and notice the Fire version still on Donald. I taunt again and then see he's hitting her. I blink (special Tauren prot warrior ability when faced with stupidity) and ignore him when he's yelling he's got aggro. Needless to say, we wipe since after he's killed, she focusses on the healer, and I can't pick up aggro this time. After we rez, he has the nerve to complain I didn't take aggro, so everyone in the party reminds him we had agreed on the order Frost, Arcane, Fire, and it was him that didn't follow it. I even tell him about my handy little add on that marks my target (in case he hadn't noticed), and he agrees to only hit the skull. None of us were surprised when he got the achievement for the normal Nexus too when we finished the instance.
Of course, to be honest, I also had my noob moments when I hit 80. My biggest was the day I hit 80 and thought it would be a good idea to tank HoL HC with 23k hp and a def rating too low to mention :P (I'm one of the guys who hit 70 after WotLK came out and hadn't seriously tanked an instance yet). Second HC I tried was not much better. I didn't even enter it before I got kicked, since I had no idea where the entrance of the Oculus was (didn't do it in normal, and now that I know where it is, I still don't want to do it, I just hate it). Of course all of those are in the past and now I'm a more than decent tank (or I can fake it better :P)

Just today I grouped with a level 68 Paladin for Karazhan.
He typed a lot of stuff in caps, and swore/nerd-raged more times than I could count.
He also demanded that enchanters shard the stuff that cannot be used by the level 70s and the shards given to him.
Late in the instance, he confessed to actually being a friend of the person who actually played the Paladin, and he was actually a known scrub on our server.

Well... on my brothers character (Mage - 80) I was doing Naxx 25.
I inspect the tanks gear.... I felt like crying. :|
He was geared Holy, specced Holy, didn't have any buffs on etc.
I told he would be a much better tank if he specced Protection and was geared Protection he would be an actual tank.
His reply was;
"but if i hav on tanx gear how will i hel?"
/facepalm

So i was shaman specced ele and made a group, one is a ret pally. We didn't get a healer or a tank so i offered to heal since it wasn't much of a problem and i ask that pally to tank, he said yes. So yeah we slaughter the monsters and arrived inside the instance. That retadin was using a 2h weapon (the epic 2h sword from ZF) as tanking and i died a few time.
I asked him why the hell he didn't (you know the question)... and he told me he was spec ret and i started to yell at him that he was the one to say yes to tank and blame me for not healing properly and was my fault.
I'm sure i had worse encounter but that one is scared in my mind.
It was both of your faults. He should not have agreed to tank with a two-handed weapon (he's not a DK) and you shouldn't have agreed to heal. Neither of your specs work out to an easy outcome.
