My two biggest ones are:
1) When people just chuck up a group invite without speaking to you at all! How am I supposed to know if I want to join your group if I have NO idea what quest/instance/general randomness you want me to help you with?!?!
2) I know I'm a druid so I appreciate that people are going to ask me to heal but when I tell them I'm not healing but balance specced they just carry on: "come on, we need a healer!" I'm a relatively new player so I'm just getting used to everything that I'm actually specced for!
I've had people go on at me over and over despite the fact that I politely inform them that I'm not a healer but do they listen? No!

People who tell me how to play my class when they can't seem to play their own.
People who don't understand need/greed. Last night was doing a deadmines run, shaman needed stuff, that was understandable, expected, it was the others in the group I couldn't take, saying 'wtf ya need for', the shaman actually only needed on the stuff he did need. If that makes sense.

Arrogant and/or rude pvpers. "I've been pvping since beta!!! You should listen to me, you all suck, wtf why no HEEEELZ HEEEEL ME (this being said by a shammie with a full mana bar to me with an empty one) Omfg we're going to lose cuz you all suck" etc.
I started pvping in earnest just so I can kill pvpers. Granted, most are not this bad but it does seem like they are the biggest tools in the game.
Actually, now that I think about it, rude people in general annoy me. Those that think that their unsolicited opinions on other people's spec or gear are not only acceptable, but valuable, or the ones that demand that others play in such a manner so as to make their own experience better. (i.e. "HAEL ME PREIST" or more likely these days, "pallies should be holy HAEL ME PALLIE!!!111oneone!")

Nope. I routinely tab out of the game to browse forums (when soloing to be sure, not in groups), then get distracted with other things (like food) and never bother to put up AFK. Sure the auto AFK goes up after about 5 or 10 minutes of inactivity, but I sure get a lot of tells between not paying attention to the game and being put auto-AFK..
Some things that haven't been listed:
- People who are too lazy to go to the instance themselves. I don't care if I and someone else is going to get there first, you should be on your way as well in case someone else is delayed. Hell, you shouldn't even
expect
that anyone is going to be there before you.
- Likewise, people who join groups and, by the time it's filled out, still aren't ready to start. These are the 'let me finish this quest' type people, but also people who just can't leave a battleground. It really annoys me when people do this and I'm not the leader, so I can't just kick them out and find someone else.
- Pet peeve: warlocks who think using SoC on single mobs is wickedly huge uber DPS. Never mind that the explosion doesn't even hit the target SoC fired from.

- People who can't stand still for however long time it may take for the rest of the raid to get ready. Ran a Kz raid this weekend where someone ninja-pulled Maiden twice - and then once more while people were running back from the previous one.
- People who threaten to leave a pug run, if they die one more time. Especially if it's a class that has it easy when it comes to grinding. And

if they cause their own death, like a certain rogue who insisted on stealthing right in the middle of a patrol path, causing us to get a tripple-pull.

Recently ran Heroic Ramps with a Holy Pally, Prot Warr, Afflic Lock, Rogue, and Me(SLink/Ruin Lock).
This lock was in mostly quest greens maybe a blue or two, definitely not Heroic material, but it was H.Ramps which is pretty easy and my dps would more then cover for her.
Reasons I thought it was ebay account:
-We can get in the instance and the other lock asks, who's healing so I can SS them?
-Randomname Green Cloth Helm of Spirit drops. Lock needs, even though she's wearing:

. I ask why and she said: that's a lot of spirit! It's much better then mine.
-Her Dps was lower then the Prot Warrior who was in tanking blues.
We did clear the instance though.

I agree with all the posts about being ready to go when you place yourself in the LFG system. You should be prepared with reagents, water, food, or being repaired before you advertise yourself. That definitely irks me. And when you get an invite you should make a beeline to the nearest flight master to make your way to the instance and not stand around waiting for a summon.
One things that drives me up the wall, that hasn't been mentioned yet, is not running back to the instance after a group wipe. It is not the healer's responsibility to come back, drink, res you and then have to drink some more. Seems very lazy to me. I've seen this from the point of view of the healer role and a non healer role. I always run back.
