If wow were realistic, everytime you started a new character you would have to travel until you could find a land that no one had reached yet, so that there would be living mobs for you to grind. The questgivers would discontinue giving quests after the first completion for the most part, because who needs the death of a loved one avenged multiple times, etc?
If wow were realistic after the first time you got mobbed by murlocs and went down it would be re-roll time.
If it were somewhat "realistic" we'd all be looking for jobs in a goblin-run factory or lumbermill, or some other boring job to pay for our food and bills sooner or later. If wow were realistic it just wouldn't be fun. That's why we all play WoW, because we enjoy the fancifulness of it.
EDIT: If realism concerns you you may be more of the simulation game type.

A better word would have been "practical" drops instead of "realistic" drops, then maybe all the trolls wouldn't have their panties in a bunch.

Great, I just wrote a longer decent reply and get the last second of "Wowhead is currently undergoing maintenance." when I try to post. Anyway, here I go again.
I am not a fan of realism in games over having a system that works well. However, on this specific subject I somewhat agree.
Though MMORPG's are all about farming a lot to gain levels and simply just spend time in the world, it would make a whole lot more sense to at least get a broken/useless (grey) item in the place of a functional/useful (white) item should that balance the game's PvE-side, since certain drops would simply be overfarmed with a 100% rate. What you see - and some of the things you don't see are what you get, that's the logic. What would happen though, is that the vendor trash would replace the normal cash drops. It would ultimately be harder on the farmer in WoW, due to limited bagspace, thus damage gameplay in a different way than previously.
Logic in loot is something that any sandbox MMORPG would take a lot more serious. In their essence, everything should be usable in one way or another, and surely it works. However, in a real MMORPG you would have forced open PvP and full loot rules from dead bodies. WoW is just too simple compared to the standard, and their winning tactics is to add a lot of 'minigames' in stead of sewing the world together as a whole like a sandbox MMORPG would.
This means: if you want to play arenas, you can do just that; if you want to grind reputation with a certain faction, go ahead; if you want to do high end PvE, nothing is stopping you; if you want to create a twinked low-level, Blizzard certainly won't help you - but they still won't stop you; if you like battlegrounds, you may travel there instantly; ... (etc). Realism in loot would in the essence be an obstacle for the average gamer, which means that I don't think it'll ever be promoted.
