Do you ever intentionally let people die?

Maybe I'm blind (or just lucky) to this, as I play DPS almost exclusively, but I rarely see people doing things intentionally that deserve this. By refusing to heal someone, doesn't that just make the problem last longer anyway? If telling the person doesn't work, I'd vote to kick and if that doesn't pass, I'd just leave and take the deserter debuff.
Problem is, a lot of the time you can't kick for the first 15 minutes, or they keep pulling, and you can't kick during combat. So it's hard to just kick them and be done with it, even when the rest of the party is in agreement.
And in terms of the people who stand in the bad, it's really about mana conservation more than punishment. A healer's biggest limitation is their mana. When someone constantly takes loads of extra damage, it drains their mana pretty quickly, and they may end up struggling to keep the whole party up. If they choose to ignore that person entirely, and focus on the tank and the DPS that aren't taking 4-5 times as much damage as they should be, then they're much less likely to run out of mana. I pride myself as a professional tank, and therefore, it would have to be extreme situations for me to let someone die intentionally. I already had to make a macro to ask hunters to turn growl off at the beggining of the run, and I use it only once, failure to comply means I'll let their pets die, and then resume tanking.
I also once had to let an off-tank die, but it was his own stubborness that led him to do that.
Terrace of the Endless spring has bosses that constantly require tanks to swap taunts (Example, Tsulong, once you have 2 stacks of the debuff that increases damage taken, the other tank is supposed to take over)
Well, I was there and there was this other tank, less geared than me, yet insisting that the mobs and bosses attacked him all the time. I would taunt Tsulong off him to prevent him from getting killed from the shadow damage and he'd taunt him right back. Me and the healers had to struggle A TON on that boss due to him, I had to spam my taunts at key moments to ensure I'd get the debuff instead of him, and healers had to pour triple heals to prevent him from dying.
On the next boss (That water elemental) we stopped caring. On Lei Shi she stacks a debuff on you that increases frost damage taken, at 14 stacks or so, tanks are meant to swap taunts, I taunted the boss off him, he taunted her right back, and I just.... did nothing, healers stopped healing him, and I stopped taunting. He wants her so badly? he can have her. He died, and then blamed me for it. Go figure =/
All I could say was "I peeled her off you, and you taunted her right back, next time share your duty with your other tank, Ser-taunt-a-lot." And then he got kicked.
He was the exception though, not the norm, I am very professional when tanking, and will do my best to keep everyone safe, even jerks or mean people.
