I normally don't mess up pronouns when talking directly to my friend, but when I'm telling other people she's coming over or I have plans with her, I sometimes mess up.

Do you think that they might mess up gender pronouns just cuz they aren't thinking of you as a woman but as a person? I know it doesn't excuse their oversight, but they probably think "friend" first then "female" later, or at least "prefers to be addressed as a female" later.

On the topic of gender pronouns, do you think that gender pronouns should be used based on physical or personal perception? Do you think that differs when it's a friend using them, or the government (or any other institution that needs to store / use gender, e.g. insurance companies)?

I'm not especially sure how that'd cause too much of an issue, especially once a transperson has changed their gender marker legally.
That's a bit what I mean though, because now you're talking about 'legally'. Prior to the gender reassignment surgery, before you would legally become female (in the case of male to female), I'm assuming you would still feel uncomfortable being referred to in the male pronoun?

Gender pronouns should 100% be based on the person's identity. When meeting someone, I try to hear how they refer to themselves, or how others who have known them longer refer to them. If in doubt, I ask if they have a preference - most people will be pleasantly surprised, and I've never had someone be offended by my asking.
Think of it this way: if, for whatever reason, you are perceived to be the opposite of the gender you feel you fit best (perhaps due to a conservative or cross-dressing event), would you be offended if someone called you by the wrong pronoun? Perhaps you might be able to laugh it off the first time, but what if that happened every day, and not just at that one event?
And in reply to the earlier post:
At what point did GLBT become LGBT?
It entirely depends on who gives the acronym - in my experience, gay men say GLBT, while lesbians say LGBT. Everyone else tends to go with whatever "sounds" right to them, or follows suit from a previous mentioning.
