THE ONLY PENALTY FOR USING A COOL DOWN IS THAT YOU HAVE TO WAIT TO USE IT AGAIN!
Well said, including the caps.

A dead tank is no use to anyone.
Given the nature of computers and the internet, you can't completely rely on others to keep you alive all the time. There are lag spikes, dc's, real life distractions (like a car driving through your house? lol), lapses in concentration, other people needing healing, slow fingers getting to buttons, tunnel visioning, line of sight, stuns and silences, knockbacks. I think you get the idea. There are any number of reasons why your healer might be unavilable at that point in time. Sometimes you just have to do it yourself.

Use Lay on Hands liberally and whenever you feel like you need it. If you die with LoH ready, the death was your fault. It is the healers' job to stop you from dying. It is your job to stay alive long enough for the healers to heal you.

There are too many replies to your post Julielou2 so I wont waste your time reading another, as I am also sure that most of what I will say was probably posted by someone else.
I am not being cocky or arrogant or any; I have tanked for about 6 years(since the beginning of TBC) as a tankadin = fail tanks on TBC (thankfully nerfed by noob Blizz), up until now on cataclysm. Tanking is a sense and so is healing, experience is greatly involved; witnessing a deadly situation go worse definitely helps you later on. But, as for basics, keeping an eye on your health bar and your healer's mana bar (tip from me; and also the rest of the group's health bars if 5-man) is what a proper tank should do. Saving the day/run, if not only considered as a well done move, should also be saluted for.
If LoH is insulting, then I am quite sure Ardent Defender is also insulting as it literally keeps you "off the edge of the cliff" ! Guildies telling you not to look at your health bar or healer's mana bar except before charging means they are pretty arrogant idiots. Therefore, /rofl @ them and leave the guild.
One major advice mate, don't suck up to guildies with their idiotic talks just because it is a raiding guild (that is if it is one), they are too many.
Good luck and enjoy your game time.

Speaking as a healer If I have f-ed up to the point where YOU WILL DIE I have no issue with the tank using LoH; guildy or not. Usually if its a guildy I am already loud in vent telling them "oh hell oh hell we are all gonna die". And speaking as a tank my screen flashes bright bloody red when I am near death... its pretty much impulse for me to hit LoH and I have only had one person say "I totally had you", but other then that most healers are big enough people to brush it off and keep on going. Chances are if you are low enough you need to use LoH something has gone wrong. Generally don't constantly watch your health or the healers mana, trust they will tell you when they are oom, but if you are about to die SAVE YOUR TAIL, it might make them unhappy but it could also save the run.

ONLY reason i can see NOT using LoH is if you have a holy pally in your raid group. You using LoH may or may not mess up the holy paladins chance to use/rely on his LoH, BoP.. In a 5 man though anything is acceptable as long as it keeps me alive and ticking.

BoP on the tank?
Also, your LoH>his LoH, and if he uses like one second after you do it just says invalid target - his cd isnt wasted.

Lay on Hands
causes
Forbearance
. Can't use LoH on target with Forbearance.
Even more of a reason for the tank to cast LoH.

If your guildies feel insulted because you are using one of the tools in your spellbook that can lead to a successful run, then tell them you have so much faith in your healers that you are going to tank without your shield equipped. Tell then you never learned Guardian of Ancient Kings because you never felt it would be necessary because you have so much faith in your healers. Tell them you are respeccing to remove Ardent Defender out of your talents so you can take a threat talent. Tell them you are moving divine shield off your action bar because you are never going to use it. Tell them you are changing all your gems to +strength and all your enchants to dps enchants because you don't need the survivability.
Where do you draw the line when it comes to "you need this for survivability" and "I'm insulted because you don't think I'm good enough to heal you"?

Simple answer here....
I have died as a pally tank, therefore, when I get really low on health, i will LoH myself.
If the healers feel like its mistrust, they have a complex. No one can heal to keep everyone up All the time... Mistakes are made.

it's not about mistrust in my guild. I get thanked for using loh in raids.
If i am low health, it takes alot of extra healing from the healers to top me up as well as keep the raid alive under heavy damage, laying takes a buttload of pressure from the healers. So they should be thanking you not crying about not trusting them. Get a new guild or let them know whos boss.
