Ooh, mine is a good one (not that all ya'll's aren't interesting; it's fascinating seeing where everyone's names come from...):
Alright, so my old username for everything was Cyberman8. I first used this when I made a Disney account to play on Zoog Disney's website back in like... '99 I think it was. Maybe 2000. Cyberman was a superhero I had made up (he was completely black, including his mask which covered his entire face, and was bonded with the Internet by aliens on Pluto), and I was 8 years old at the time.
Anyway, I used Cyberman8 for everything until I signed up for the Nintendo NSider forums. NSider used a username to log in, and a screenname that everyone saw (that way it would be much more difficult to hack people; you couldn't just put in their username and guess at their password). So I used Cyberman8 for my username, but didn't have a screenname thought up.
Well, a few weeks earlier, my and my brother had been playing a live-action game (good old fashioned backyard make-believe) based on the Metroid universe (me having been in my Metroid phase, hence the Nintendo NSider forums). My brother was playing as the human Galatic Federation trooper raised by the Chozo, Tana. I was playing S'kree'ran (pronounced Skree-ron), his elder brother who had been captured by Space Pirates as a child and whose DNA had been bonded with Space Pirate DNA, making him a hybrid.
So finally I chose to my S'kree'ran my NSider screenname. I ended up using that as my character's name on Runescape, but it didn't support apostrophes, so I ended up using Skreeran1 (beats me who got "Skreeran" before me) and eventually made Skreeran my first character on World of Warcraft.
So here I am now. I've abandoned the old Metroid character, naturally, but I'm still fond of the name.

The description for mine is in my user profile, but I'll copy/paste it here anyway.
A couple of factors converged to make this become my internet alias.
My real name is Donovan Murphy. However, no-one has ever referred to me as Donovan; at least not for very long. I've had about 1000 nicknames, although the vast majority of them have never stuck. About 95% of people know me as "Donny"; although I have one particular group of friends who call me "Donson", and for a short while they also called me the extremely lame "Donson & Hedges", back when I smoked Benson & Hedges.
Anyway, a guy that I was drinking with quite a lot for a few months immediately after I turned 18 coined the terms "DonMurph" and "DonnyMurph". They never stuck - only he and one other drinking buddy called me by those names, and they were faded into disuse within about a month.
However, not long after that, I remember that I had to provide my email address to someone; I think it may have been a potential employer. My internet alias up until that point was "importaskyline" (from my love of the Japanese performance car - I'd had it since I was 11 or so). I realised that "importaskyline" was not a very professional-sounding email address, so I had to create a new, more professional-sounding one. "Donnymurph" was still reasonably fresh in my mind, and so the legend that you know today was born!
I still use "importaskyline" for eBay - there's no way I'm sacrificing all that hard-won positive feedback! I use "donnymurph" for almost everthing else; I've never ever found a website where the name has been taken, making it the perfect internet alias!Squishy's a high school nickname, which got converted to Squishalot for any fantasy-based character.

:P my internet name I use the most for almost everything (Youtube account, email account, shammy in wow) is Shakduhn, in fact I wanted to make that my wowhead username, but when I actually made my wowhead account I wasn't aware that there were several realms in wow xD (I used to play wow in a private server U_U yes, i know, disgracing, but I wasn't able to aford it back then)
so, I made an account here, but since I held the hope of one day playing in the retail realm, and I didn't wanted anyone "stealing" my name, My wowhead account is Rankkorr, wich is just how you pronounce in spanish the word "Rancor" a creature from the Star Wars Universe. (the R is more firm in spanish, and the C is more like a double K in spanish due to how it rolls out in our accent xD)
Later I knew that there were in fact several Retail realms, so even if a dozen people stole my name, I'd still be able to make my shammy on several other realms. =/ bummer.
Everywhere else I just use Shakduhn as my name (I'd change my wowhead account name, but i'm too lazy to even request it, and everyone knows me as rank anyways)
as to where does Shakduhn comes from, that's a funny story, as back when I was in 8th grade, as a project for spanish class, I translated a full Shakespeare novel into spanish (easier said than done, believe me, old english is weird, old spanish is weirder).
The rest of the school nicknamed me "Shak" from that point all the way till graduation day :P so I used it as my username for all sorts of games, on Counter-Strike it was Shakshooter, on Warcraft 3, it was Shakrazor, and eventually, on my first MMORPG (Lineage II) it was Shakduhn, (Since my class was an Orc Destroyer, and in the in-game lore of that game, the greatest destroyer was Duhn the grey).

My primary Internet username, OverZealous, originates in two stories.
Back when I was like ten, one of my teachers took a liking to calling me Oz for no apparent reason. He told me much later that he had always thought I reminded him of the Wizard of Oz. I have no idea why in hell he'd think I remind him of a weird wizard from a fairytale, but Oz kind of stuck.

comes from Oz, since many websites require 4+ character usernames. I was considering OzOz at first, but I thought that sounded lame - so I went looking for any words with O's and Z's. A (WoW)guildfriend called me Overzealous in my desire to find the perfect username, and heh. You could say he nailed it right there.

My first time I played WoW was on a private server on a friends pc (also my first time playing WoW). I didn't even know that it was one or even what a private server was until many years later.
Happily playing retail though!

I used Curse and Dysfunctional way back in the late 90s, they were my 2 primary name with Dysfunctional as my main counter-strike name. My friend suggested Murrdurr for my rogue toon in Everquest when I decided to roll a rogue in '02. And not much long after that I used Insomnia also, I worked afternoons and played a lot at night and when people in my guild who were on PST saw me on they always asked if I had insomnia heh.
