So, I'm thinking:
For the new raid on Sargeras... maybe 50+ man raid, harder than AQ 40 was when it first came out... the ultimate test, no screw-ups allowed.
Bosses:
Sargeras
Kil"jaedan
Zmodlor, or maybe, again, in a dungeon/heroic dungeon version of it
And some other names that aren't from lore that we aren't supposed to know about, with unknown races. The Burning Legion corrupted countless worlds... so how could we know about all of those races?
I think the Sha'tar should also make a comeback.
Edit: Oh, and maybe one more round of Doom Lord Kazzak too. That'd be nice... to have him as an indoor raid encounter. I think in the hardest raid, he could be an exceptionally tough boss. Not that the others wouldn't be.

No screw-ups meaning that if you mess up, you die. In other words, it'd be the hardest challenge in WoW (we are facing the strongest being here).
Yeah, I do agree that I don't want a whole bunch of rehashing, but Kil'jaedan is kinda needed-- he's also a major part of lore. And we never killed him. Technically.

Aye, at some point the last fight with Kil'Jaeden will happen...and if we're lucky, Velen will be there to help us take down his old friend.

Agreed.
Also, I play Horde, but I can't help wondering:
With all of the lore, why is Velen the
weakest
Alliance boss? Practically everyone and their newborn son can kill him. Unless they made some crazy changes in Cata...

I didn't mean that. I mean that in fights, there's even less room to screw up then there usually is in raids (which still isn't much).
Edited for grammar perfect and goood zspllng

I think that needs to be done anyways. At least a quest like this:
Raid tier one:
Raid A
Raid B
Raid C
Raid tier two:
Raid D
Raid E
Raid F
Raid tier 3:
Raid G
Raid H
Raid I
Where a quest to even grant access would say something like:
To get into (Raid tier 2 raids), complete at least 2 of the following (Raid tier 1 raids), and the same for raid 2 for raid 3. This would prevent newbies from thinking they could take on any raid (I once had a healer join up for ToC 10, and found out after a few wipes she was wearing almost all greens, a few blues, and a purple)...
