I was never able to get past the Troll boss in ST, so I never got to hakkar, or the big dragons. They just made it way to dependent on other bosses. So, in order to kill this guy, you have to kill this guy, and to kill him you have to take down the shield by killing him, him, him and him.....

Of the early ones, Gnomeregan is probably the worst. The dark iron mines, the aggro ranges, diseases, party-killer rockfall event, length, spell-immune mobs and alarm bots all just add up to too much of a jump from Deadmines and Blackfathom Deeps (both of which are decent instances imho). Uldaman later on is long and the level range is screwed, but it's not a bad instance. Stockades sucks as well, it has bad loot, many easy-to-aggro hidden mobs and is dull and uninspired on top.
Later on, Blackrock Depths is just too damn long and relies too much on must-have classes, which made me hate the place, although the encounter design and ambience are ok. Sunken Temple is also above-average annoyance levels, because of the patrols. Mostly though too many instances in the 40-55 range are too far away from the population centres - Maraudon, Sunken Temple, Dire Maul, and Zul'Farrak are all miles away from Ironforge / Stormwind / Darnassus, and they're not a lot closer to Oggrimar / Undercity.
I don't particularly dislike any of the Outland instances... the winged, short-run designs and the ability to fly straight to them just makes it a lot easier to do what you like.
