Let me start by saying I, like many others, have absolutely no idea how did I get my account compromised.
I mean, constant virus checks were conducted and I never did anything stupid the likes of which could get my account compromised, but hey, it happened so whatever...
So, my wow account was stolen (To be precise, Battle.net acc) and an authenticator has been placed on it.
I found out about the theft when my account was banned for 3rd party tool usage, and during my check I've found out through my armory that my character had 450 Mining and 450 Herbalism, even though I didn't have WoTLK but used my trial up already (If anyone can explain me how can one pull grandmaster on any proff without the help of WoTLK I'll be really happy).
Now, after a few days I got the authenticator removed, and in a few more days I've recieved a request for World of Warcraft Account Recovery Form through an email.
Problem is, I have a different name on my Wow account name then Battle.net (Yes, I know, I am very stupid for using a fake name on the wow acc. ) however, the battle.net account personal information is legit and confirm-able through my drivers license.
Now, my question is:
When you send in your ID, do they check the name on wow account or on the Battle.net account to authorize and approve the unlock?
What happens if I'll send them my id and they will check the wow account name which will show up different?
Could I say I changed my name? :\ (Cheesy I know)
At any rate, thank you for your help. I really just want to play my account again :(

They may require you to correct the name and information.
They will know where in the world the account has been accessed from.
You might have to provide photo-copies of all identification material to prove the account is yours.

Errrrm... I've got the boxes on my hand with all the CD-keys, but like I said my real name and the name on the Wow account differs. Since I couldn't find the option to change the registered information (Name, last name and phone) I presume they are very strict regarding one time registration and no further possible change. Is it so? :\
And what do you mean they'll know? I played from Canada all my life, didn't use proxies and I am pretty sure my ISP doesn't use any...
Also, are you saying that they might request more then a driver's license to prove my identity as the owner of the account? (Also, would Canadian driving license work?)

If you have the CD-Keys, thats proof enough. Just correct your information as soon as you can.
They know you always connect from canada, so if they show that recently there has been a connection in China (as an example), then they will know right away the account is likely compromised. They doubt you travle from Canada to China then report it stolen. HA
All you need is your CD-Keys, and personal ID, i belive.

Yeah, they DO know the account was compromised, and all they're asking for now is just the form.
My only concern (like I said before) that when I was creating the world of warcraft account (Since it was trial and all, I wasn't really planning to play, but then made a few friends there and decided to stay) I made a fake name something like Warhead Nukes or something like that.
Obviously there's no proof of such human being at all, but I DO have my real name on my battle.net account which is the wow account on (They are connected/merged)
So just answer this question:
Will my ID (Of the name on the battle.net account, not Wow) and possibly CD keys suffice to return the account to me?

I don't know what sort of name and info they can ask me to correct if they don't give the option online (So what, if you move per say you have to send them papers confirming that?)
And how should I go on about sending them the CD keys? The form doesn't have anything on it where I could place the CD keys...
Oh, and how can I fill the form without printing it out and scanning as a picture as I am quite sure together it'll end up more then 3 MB...

Call Them. its much easier. They can answer all your questions over the phone and fix the account right away over the phone. I posted the information above.

Alright, then two last questions:
Is it long distance to call from Canada to Blizz in US?
And
Will I still have to send in the form after calling them?

If 800 numbers are long distance for Canada, then yes. In the USA, an 800 number is typically a toll-free call.
