Personally i don't pop more then one of my cooldowns at one time, unless it is a dire situation meaning i'll die if i don't, you should save the cooldowns for the bosses are hitting extra hard then usual, like festergut and his inhaling.
I don't really count that 6400 damage shield from the trinket as a cooldown, since the damage it absorbs is small, but, every little helps. If your tanking a raid, ideally you would be blood, indeed, blood tanks are harder for bosses to burst down thanks to our WoTN.
The spec itself is okay, it's a little different from cookie cutter, as it were, although whether rune tap is useful is a moot point. most tanks would spec hysteria and probably scent of blood aswell. You wouldn't use it on yourself, perhaps one of your good physical dps, you could use it on yourself though, although since it drains health i wouldn't reccomend it. I don't know whether tanks use Dancing rune weapon these days, it does add to your threat, though, it isn't affected by frost presence, i don't use it though, so, i could be wrong.
Glyphs are a bit weird, Dark death isn't that good, rune strike or vampiric blood would be great, why not get both?
Personally i don't use any tanking addons except for omen, and DBM (although it is not a tanking addon, but still, very useful.)

Talents:
Your spec is pretty okay, its a little different from the cookie cutter but those are only fillers. You can move the points from RT and IRT to 2/3 SoB, 1/1 Hysteria(for on the dps) and 1/1 DRW(for extra damage and threat).
Cooldowns:
Whenever available during a bossfight, or when you know a lot of damage is coming - check the tactics for that. Aka, at 100 blood power at Deathbringer or so.
Gear:
Enchant your gear. Most pieces arent enchanted, and the def enchants are not that good. Get stamina/health enchants. Also, i'd try to get the

from toc - its alot better than the BBC.
Addons:
I really cant advice you on specific tanking addons. Im using 47 addons total, and all have a purpose to me. You might think different about that though. Check curse.com for tanking addons, and try them out.

Whats the difference? I really like the fact that with some common sense and the addon you can run an encounter you overgear a little(i.e. Ulduar) without ever knowing tacs. And its great for real bossfights too :)

I just need a good, clear, prominent indicator of stacks of whatever debuff is on the other tank and me. I do OK with my debuffs and the addons I have, something that highlights the other tank's stacks would be nice.

It may, I just can't find it in there, I will keep digging but if anyone else has any other suggestions, it would be nice because it is often hard to pick out stacks on the other tanks debuffs, even when I keep them on focus and sometimes, it is hard to have them tell you as well, vent or not.
Just looking for one more tool to reduce my chances at being uber fail tank.

You only need to activate the "show debuffs" option for your target's target, in whatever unit frame you use (xperl, suf).
Another very nice addon i'd recommend is Tidy Plates with the Threat Plates plugin, very nice to instantly recognize if you lose 1 mob during aoe pulls, if it happens the nameplate turns red.
