I'll give an example of what I mean:
I've been in raids where, whenever someone dies, a message pops up in raid chat, eg
Donnymurph:
Autopsy: hits Malgayne for 13136 (952 O)
Where is the name of the boss mechanic, Malgayne is the name of the player that just died, the figure in parantheses is how much it overkilled him by.
I assumed it would be called "Autopsy" but I searched for autopsy on both Curse and Wowinterface with no result.

If it's just to satisfy your personal curiousity and not to be used as a brow-beating tool then Recount already has that built in. Each death is on the "death" track and is accompanied by the last several seconds of the log associated with that player.
So has Skada, although Recount has more parses.

The reason I want it is, as a raid admin, it makes a good educational tool for my raid members. Thanks for the link Sjak.

Another one that is stupidly easy to use is Fatality.
It only works in raids however.
