Murmur was the 'Lord of Sound', but we've only ever seen one Old God(possibly) outside of Azeroth, and that was what the dead arrakkoa of Shadowmoon Valley were trying to summon to Outland.

If they are going by a little bit of the standard DnD universe for ideas, then there are different power levels of gods. At the top would be the Titans as they seem to wreck anything that stands in there way.
Next would be the Old Gods of Azeroth, there could be others on other planets these are just the ones we know of.
Also with the old gods would be the Demon Lord Kil'jaeden.
The next level would standard god level, this would be the Earth Mother, Elune and such.
The next as far as I can tell would be the more active god beings like the one that the arrakkoa were trying to summon but also beings like Hakkar and the animal spirit gods that trolls worship and in some cases enslave for power.
After that would be the demi-gods like Cenarius, possible the god of the quill boars would be a demigod or maybe up there with the animal gods. Not sure he is as powerful as say Elune and such.

If there really is an old god underneath Maelstrom, perhaps it will be that if you destroy the other old gods he will rise and wreak havoc. Like in various books I have read where the heroes kill several main evil characters, however in killing them all this causes an even worse one to surface.
I still like the pentagram idea though, it seems quite likely that the old gods really are in locations in the shape of a pentagram, so it would not be that surprising if one was right in the center of it.

The Old-God from the Maelstrom will be reserved for a later expansion because at the end of Stormrage it was pretty clear that the force behind the Nightmare was indeed an old god, but I don't think that they will waste that on an expansion whose main antagonist was pretty epic. Yogg-Saron was very tied into Northrend (saronite, corruption, etc) with most zones having some kind of suggestion to his presence. So far, as I can tell there has not been much mention of old gods apart from some quests in Darkshore (correct me if I am wrong).
There is much potential that the Old-God beneath Lordaeron will be revealed with a tie-in with the Scarlet Crusade and how they were affected adversely from being in such close proximity to it.
I doubt this very much tough, because generally (so far), an old god has been introduced every second expansion and they may keep on following this trend. It would make more sense if the Nightmare that was left at the end of the dream and at the start of the great beyond did start moving back into the dream, warranting the Emerald Dream expansion pack that everyone (seems to) want.
Overall though, this is all pure speculation and an Old-God could just as easily pop out of one of the rifts that Deathwing creates when he blows up everywhere - we will just have to wait and see.

My wording was a bit off, I wasn't saying it has happened but they could tie it in easily enough (i.e. it made them more susceptible to corruptioin, etc).

Except the Crusade is gone, wiped out in Vanilla and Wrath; they are no longer a real threat. And they don't need to tie it; they did that with Garona and Yogg... which is such a load of crap, as the Old God of Death had nothing to do with the Shadow Council.

That is a "something" the Arakkoa are trying to summon in Shadowmoon Valley. A quests call that thing "an ancient, powerful evil" and it is the only indicator of Old God-related activity in Outland.

Lets see. We have the old god affecting the emerald dream but that will be its own expansion.
We have azhara's Old God and that may be the next one, but i think she's getting her own expansion, along with her old god.
All the old gods had a hand in Death Wing, since they were chained in his domain.
I think most likely, if there is an old god, it will be someone we havent seen yet.

He was a member of the Shadow Council, therefore the Old Gods had (limited, although it may have spread) influence over the Council, which was responsible for Garona assassinating Llane.

It was sill Gul'dan alone who made all the decisions; everyone else was used as just as much as Cho'gall was, and the Ogre magi was Gul'dan's second, and wouldn't do anything without his orders. The Old Gods probably got their hooks in Cho'gall at the Tomb of Sargeras, something similar to Azshara at the end of War of the Ancients; they saved him being ripped apart by the packs of feral daemons in the Tomb and got an instant servant.
