I think the appearence will be something like
this
, or just something that looks like an amoeba. Either way, I belive it will have no definate shape - based off of the new minion of the Old Gods that is appearing:
the shapeless
. The equivilent to the stomach and minds eye for this one could be his heart, which you have to beat to death in order to weaken him.
Funny you should mention that, because Yogg-Saron, called the Fiend of a Thousand Faces, has minions called the Faceless. So it could be the exact opposite.
Edit: I like the idea of a hand God. Kind of like that monster thing in Cirque du Freak...

I picturing this one being called "The Thing that Should Not Be" or something similar to that - even thought I know that originally reffered to Yog-soth'oth (A.k.a: Yogg-Saron).
EDIT: And the limb is interesting.... but what would you beat senseless? It's thumb?

Mouths nor eyes don't have brains. So, the hand creature could have a brain just as easily. Actually, having to defeat each one of it's fingers before attacking the brain or whatnot Kologarn style would be pretty cool.
By the way, "The Thing that Should Not Be" from Metallica is Ktulu (Cthulhu), because it says "He watches, the game, beneath the sea!" Though this one could be aquatic octopus (but even more) like one. Might have a shapeless general that looks like Cthulhu.

It's "He watchs, lurking beneath the sea," not "the game." Trust me, I've listened to that song constantly.
EDIT: But you make a valid point. Not sure if Blizzard would trade what they've done with the Old God fights so far for one based around Kologarn, though.

Woops. Don't know why I wrote that... Typing too fast. It's not one of my favorites, I prefer Leper Messiah, Sanatarium, and Disposable Heroes.
Edit: I meant Kologarn as in Fight Style, except with a Brain type 'real health' thing, not as in shape.

I like the Star theory a lot but I just don't know where they might be. It seems to me that the Old Gods were originally one creature that was divided up into different parts and so I've always wondered if C'thun may be one of two eye creatures. As for organs we have lungs and a heart left. As for the Old God that was supposedly sealed beneath the Well of Eternity IF there was one I would think it was the body. It seems that the Titans placed the Old Gods at the very ends of the continents as far from the Well as possible and so I would not be surprised if they hid the Old Gods' body beneath the Well. Perhaps without it they're immobile to some degree. As to which Old God will show up in Cataclysm I'm not sure. Personally I think it'd be more humorous if the Old God to show up in the Emerald Dream were the nose and the Snoring is what caused the Nightmare. It seems to me that the Titans left each Old God with an Organ unrelated to the part of the face. Like how C'thun got the Stomach while Yogg'saron got the Brain. So I would say the Nose would not get the Lungs but probably the Heart. Even if the idea of a Nose and Ear monster sounds laughable I wouldn't doubt Blizzard's ability to make them seem formidable. Although I can't for the life of me imagine what the heck the Ear would have for abilities. Ear wax anyone? If there is another eye creature then perhaps they'd just skip the Ears altogether.

.So in the beggining they where seperated.But i like the idea about the organs.Sooner or later all 5 Ancient Ones will be free and i think there ultimate goal will be to combine with each other and we players have to stop it.Another think that came to my head a few days ago.I was thinking of this raid in the Well of Eternety where all the bosses will be diffrent parts of the Old God in the well.The final boss the head will be hard and we have to kill it so we dont allow the Ancient Monster to fully realease itself.And the mobs...Naagas,Faceless Ones and Twilight People

Actaully i like it.Something like when its tenticles arise from the ground insted of grabing us they could transform right infront of us in some horror unseen.Or this fiend of thousand shapes can hypnotize us into seeing our close ones (faction leadders/raid members etc.) shapeshift inside out into something revolting..thats will be the "crazy" part of that Old God encounter.Or the Old God himself can transform to eyes like C"thun to mouths like Yogg-Saron...i rly like your idea.
Btw just yesterday i tought of this palm/hand monster.He will be known as the Palm Of Terryfing Horror or Death"s Grasp.Any toughs?

The star theory sounds interesting but you are all forgetting when the titans sealed away the old gods, there was a lot more land for them to be sealed in. Remember when the Well of Eternity blew up it took the center part of the world with it and sunk a massive piece of land beneath the waves. About 80% of the land mass, so we went from one massive continent to 3 major ones and some smaller. Maybe something like what these people put together.

So there could be old gods buried beneath the waves. We do know that at least one is responsible for the naga, so that could be the one we fight in this expansion.

And the waves were only formed after the Well exploded... and again, why would the Titans seal a creature of infinite evil under a source of unending power? Yogg stretches all across Northrend, it wouldn't be that hard for the North-West star point Old God to have been to reach out to the destroyed Azshara castle and offer the Highborne eternal life.... hell, the Silithid burrows are all across Southern Kalimdor, and they are the shock troops of C'thun.
