Hi,
We did this instance, but we couldn't figure out what to kill to get the Satchel and complete it. Anyone know? Thanks.

For upper city runs you need to kill the emperor. I'm not entirely sure, but I believe you need to leave the princess alive in that encounter to get your satchel reward.
I don't think so, I've done that place once or twice recently, we usually Princess and always got a bag, more that likely either a glitch in the system, or you were actually assigned Lower City or Prisons or whatever they designated the one-boss run as.
