Vanish is eternally broken.
It works best in PvE but sometimes still fails there.
I've had similar issues when there are several melee mobs that a last attack off of them will bust me out of vanish right after I hit. Usually I don't have issues with a successful stealth and then still being chased by mobs but, then again, I'm not sneaking around opposing faction cities often either ;)
In PvP it doesn't always cause a drop target like Misdirect does, spell effects in air and some still being cast can still reach you and break vanish, and pets usually continue to chase you eternally.
Fixes by Blizzard are always in consideration but they're worried on making the fix too strong as it would make vanish stronger and maybe too strong. So we have this ability that works sometimes.
Many of us burn CoS to up vanish's reliability as incoming spell effects then may not do damage and thus leave us in stealth.

Edit: Jaguarz's vaguely related post might make you think this is a bug. It's not. City guards are hard-coded to ignore vanish.

The emphasised statements don't really match up.
My experience with guards, usually guards at faction controlled camps or villages rather than main cities, matches with KidB's statement about gaining some distance before vanishing as the closest guards continue chasing with the further guards seem more likely to break away (before their normal leash distances).
If this observation is true then the guards are not ignoring vanish.
They may be treating it differently than other npcs or have an enhanced perception stat that makes it harder to vanish successfully but vanish is having an effect.
Vanish adds 370 points of stealth (74 levels) onto your character and distance makes that effect even higher (as is normal) so characters further away dropping off makes sense. In normal situations characters close to you should still lose you since it adds so many levels unless they see right through stealth like detection units do.

Both view points are stated, neither are modded highly or confirmed.
According to observation that is not disputed:
- Guards will break away after a vanish given some distance.
- This distance is
not
the normal break away distance you would have simply by running.
This does show that guards are effected by Vanish in some way.
My guess, following the above information, at the moment is that they ignore the bonus stealth granted by vanish and treat you as a normally stealthed unit.
Guards far enough away will then break but guards right on top of you will see you because they're facing you and are close enough.

The guards chasing u even after vanish has been in the game for a very long time. Bliz had done that change bc rogues were pretty much the only class that could get away with pulling stunts off in major cities. It was an effort to even the playing field and blah blah blah
I remember when they patched that in...I was a very sad panda

I don't know if they changed this, but back in the beginning of wrath, if you killed a player in an enemy city or town, that player's corpse would spawn a guard :|
I stopped ganking after that.

-Guards w/stealth orb above their head. - They can see you from a long ways away. If you're a subtlety rogue and it's inevitable this guard is headed towards your location, you can shadowstep and cheapshot him before he sees you. (just have to make sure you're almost the full 20yds away)

Well, vanish isn't
completely
useless against them. If you can get enough distance (good luck, they like nets and stuns as I recall) it will drop you into stealth and as long as the distance would be enough that they wouldn't see you in
normal stealth
they should break. About as useful as looting bodies in PvP for the gold, I know.
Why Shadowstep and Cheap Shot? Why not Shadowstep and Sap (or are they immune?)? I'd thinking fighting guards like that would be a waste and give away your position most of the time. Ganking people or commonly used npcs is the goal isn't it?
Haven't snuck into a major city in a long time now, maybe need to relive a few old memories and curse out the 'perception' guards some while i'm at it.

Probably not Sap because they will detect you instantly and auto attack you so your best bet is to just stun lock them down
Shadowstep should have put you in their blind spot.
Do stealth detection units also lack blind spots?
Most of them
do
come with dogs, of course.
I'm referring to the ones that don't or if their dogs were killed ie: the humanoid patrol mob itself.
If you're jumping into a group of enemies, even behind one of them, it's very hard to not fall into a detection arc of at least one of them as everyone moves so in those cases, definitely just open and go all out. I'm just looking at the principle of 1 guard, why not bypass it?

Stealth detection units ignore aggro. They consider you not stealthed.
