Kicking people out of raid

That is not a reportable offense. You can remove anyone from any raid with or without a reason.
I could not tell you how many times I have seen people removed from raids for whatever reason. As far as people like the one you described, nothing that bad. I have seen people pull a boss or start an event when they were removed.
After a wipe caused by low dps I pick out the lowest 2-3 dps and tell the raid leader they need to be removed. If they will not I tell him to replace a few dps or repalce me being a geared healer. Sometimes I can get my friends in the run.
When I get people removed some of them send me tells, I tell hem to run heroics and set them on ignore.

Doing top dps, last person to die (Read:Never stood in fire.)
We wipe on the Flame Watcher.
RL: "Not enough healing"
Healer:"I could use somebody helping me. We sohuld have two healers."
RL: "Oh yeah."
*kicked.*
Me pst; "WTF? I was doing top dps!"
RL: "We need another healer."
Me: "Kick the one doing 800 dps he's the one bringing us down...
RL: "**** No. I kicked you. Deal with it."
...none of them were in the same guild or anything...
Edit: Just realized this is about you kicking others. Disregard this.

I guess my experience tonight can go under this topic.
So my standard Tuesday early-morning is to try and find a group for the raid weekly quest after it resets for the week but before the raids restart. Last week's was Razorscale, and this week was Ignis, so most everyone on the server was already saved to 10 man. I end up getting into a full pug 25 man in trade.
So once the group gets kind of full, the raid leader sends out a /rw saying "Hearth to Dalaran and fly up here. I can tell who's hearthstones are on cooldown, so if you don't hearth I'll find someone else."
I'm thinking... no, no you can't tell what my hearthstone CD is. So I call him out in raid. A little bit later he asks "Okay, who's not yet in Northrend." I shoot back "So you are saying you can tell what our Hearthstone CDs are, but you don't have raid frames to tell you where everyone is?"
Guy whispers to me "I'm just bluffing to get people here. Stop making me look bad by calling me out."
Fail bluff is fail.
In the end he didn't kick anyone, but I thought it was appropriate: "Raid leader bluffs about having an addon and threatens to kick people."

I cant help but thats my experience.
And I wouldn't even do Ulduar, as it requires a lot of coordination and understanding of boss mechanics.
