Nitpick time! (just some extra info).
In Violet Hold, the multi-mob portals are 3 mobs for waves 1-12. After boss #2, they become 4 mobs.
Also, Cyanigosa doesn't tail swipe.

A couple things I noticed:
You sort of mention this, but aren't specific about this particular room at all: I've seen ICC 10 geared tanks die in the first room of UK HC. 7 mobs + all enraged + 5 stacks of sunder + stunned (so only ~9% avoidance from miss) = dead tank, at almost any gear level. I would explicitly point out that that room requires some careful pulls.
In Utgarde Pinnacle, Skadi does target someone random for the whirlwind, but he
can
be taunted, and he will start whirlwinding in your direction. Healing one geared tank through that damage is trivial, healing two clothies is anything but. If I was you I would recommend tanks taunt Skadi if he gets close to anyone but the tank.

Nice job! Wanted to add, on
Crusader's Colisseum: Trial of the Champion
, the adequate order is Priestess > Lightwielder > Monk.
Reasoning being, Priestess because of the Mind Control / Heals, Lightwielder because of the Unbalancing Strike and AoE/Heals, and lastly Monk because he is not a threat for a tank and is the one casting the AoE slow when killed.
On the guide, the Lightwielders are suggested to be killed last, but that's because they're mistakingly identified as the casters of the AoE slow.

also, quite often in TOC5, after the horse stuff is done, the Grand Champions can reset on the pull, and i still don't know what is the reason for it. any info?
The reason for the random resetting is usually a line of sight issue. The gate blocks line of sight (there's an invisible wall there), and if a mob attempts to cast at the start they will fail, and despawn.
I'd still recommend charging, silencing or deathgripping the caster(s) to ensure they have targets in range and line of sight when they try to cast, but in the end it's more luck than anything.

Just two things about Trial of the Champion:
The Grand Champions despawning is caused by the defeated jousting versions of the champions not properly getting up and running into position.
If, once the Grand Champions are ready to be engaged normally, you see one of them still lying on the ground, it's going to reset.
Also, regarding Paletress: she actually doesn't attack at random while her shield is up. She is perfectly tankable, and should be. I've seen many, many wipes on her because the tank assumes she doesn't have a threat table, and she's left to her own devices, smiting random DPS, but more often, the healer. Those smites hurt.
She might toss out a random smite now and then, but she does have a threat target, and the bulk of her smites will go to that target.

Iralima: Many thanks to you again for putting in the time and effort required for a guide of this caliber. My paladin has been tanking heroics for a couple weeks now (off-spec), and I seriously would probably have geared his second spec for retribution if it hadn't been for your guide. Now I'm grinding out badges for BoA gear with instant queues, something my mage has never seen. For anyone I hear proclaim "I'm afraid to try tanking," I send him straight to your guide.

While I've found this works, he usually picks a new target before Hand of Reckoning goes off cooldown, so I end up using Righteous Defense too. Other tanks with only one taunt (not counting taunts on a 3 min cd) might not be able to do this as effectively, but taunting once is better than not taunting at all.
The trick is to stand as far away from your DPS as you can, and taunt when he's almost at them. He'll spend 5 seconds going towards them the first time, four seconds heading towards you, and then four seconds moving back to them. He'll probably get a swing or two on your DPS, but that's not a big deal.
