We were doing Shadow Labyrinth, and the party was poorly-specced, inexperienced, and under-geared. This was a guild run, and all the participants but me were firmly in the "casual" category. They'd played a fair amount, but mostly only soloed. We persevered, but it was by far the most brutal run I've ever done. At the very end, we managed to fight our way to Murmur, and we got to the true goal of the run: the Karazhan key fragment. We arrive, I am relieved it's all over, and one of the party members opens the container. After we kill the guardian, the three other party members say, "Oh, I didn't have this quest after all. I guess we have to do it again."
I almost cried.

This is another Prince story. The raid was doing fine on him then the tank opens his big mouth and said, "We got this." All of a sudden, infernals drop at the worse spots and raid wipes. After that, we pretty much told the tank to keep his mouth shut until the thing is downed.

One of my worst times was during Vaelastrasz attempts with my guild. I usually keep a list of the items I'm looking for on a small notepad that I keep on my desk with me. While we were waiting for a few raiders to come back, I was penciling in an item, and when I went to set the pad down, I fumbled with it and hit (of ALL the keys on my keyboard) Mind Blast. Needless to say, I felt terrible, and didn't let that happen again.
Although when we looked at it in retrospect, we found it funny how quickly Vael could demolish 40 people when he's not properly under control.

During our first runs in kara many many months ago there are those AE packs at the top of the main staircase with the elites pathing... while aoe'ing one group I was just seeding stuff away and of course I mad the ultimate mistake of the TAB target... naturally I targeted one of the elite pathers who brought with two more groups of ae's - there was a positive side to the story, the OT was smart, picked up the elite pulled him away ae burned down adds and we killed the elite - and two trashey epics dropped. So I went from zero to hero :)

Last night we were clearing Karazhan to Curator, headed in the side door. My paladin got there early, and the mage, hunter, and one of the rogues joined me. We were waiting for the rest of the raid to join us. One of the three--I didn't see which one--decided to go through the door and take a look around. Possibly the rogue stealthed through, and one of the others followed him without thinking. But regardless, the first Trapped Soul aggroed, and killed the hunter and mage. I used Divine Shield and managed to get back out the door before they got me too, and presumably the rogue vanished.
So that's irritating, but fine. I came back in through the door and tried to rez the mage and hunter. Except I couldn't do it--every time I would open the door, begin casting the rez, and before the rez was complete the door would slam shut and I would get a "target is out of line of sight" error. So I snuck out the door, and stood right outside it (just out of aggro range) and rezzed them from there. The rogue goes afk, and I go back in through the door, not wanting to take any chances. Except after the rez, they're staring at the door. They assume they're on the SAFE side of the door, and start walking. Instant aggro, and the mage, hunter (and the poor afk rogue) die AGAIN. We had two wipes before the raid even showed up. =/

We were trying a "newbie" run through Kara, with a very mis-matched group. We were on our second attempt at Attumen (we ran out of time the first try, and got caught by respawn), and were working on the last group of trash when someone, somehow, managed to aggro Midnight through the wall. *headdesk* We called it a night after that.

.....um aggro midnight through the wall??? There is no way on in the world you can aggro him through the wall.
Welcome to Seed of Corruption. It's finally getting fixed in 2.3, but currently it happily blows through any and all walls.

you have to attack him to get him to aggro so unless someone glitched it up and had an attack go through the wall you can aggro him.
That's exactly what I mean. Either one of our mages used an aoe that glitched through the wall (not me, I don't use aoe in close quarters fights with cc or other mobs nearby), or more likely (as suggested by other commenters) our lock's Seed of Corruption. Shame no one checked the combat log, I'm curious. Also I'd like it to not happen again. :-p

Having someone come off the teleporter platform auto-running straight into Sapphiron while we were setting up a pull. There is no escape.
