The cast itself appears over the hunter. Had me freaked out the first couple of times whilst raiding. I have enough threat issues as it is, and here I was thinking that one of the other hunters was playing silly! :P
But it does work, no symbols appear above the tank as far as I can tell.
On another note, having 5 hunters in a raid, cast MD at the start of a boss pull = instant 250k threat generation for a tank! We surprised our warrior tank last night whilst going doing Saurfang.

What i used to have and didnt have to type in the name everytime...
/target focus
/cast Misdirection
/targetlasttarget
This was awesome beginning of a Heroic or Raid I select my Target and set him/her to focus.
Now that same macro cast it on me..just asking what changed where this doesn't apply it to my initial target as it once did.
It selects my focus target but puts Misdirect on me...weird.
I've always had bad luck with lasttarget in macros, half the time it gives me some random guy, sometimes it even gives me a random friendly person in my raid.
Even easier macro than all the ones listed if you don't want to have their focus set as their tank.
/cast Misdirection
