But do you honestly believe everyone's gonna be honorable? Also, not everyone has a set of dice. (I do though) or a manual.
Also, Lore/RP covers the bases of D+D mostly. Just go there.

The idea was raised before, but it would probably take too long.
I've got no qualms with a code of honour. We could probably cross-check die rolls by randomly sampling and seeing if they're abnormally high, statistically, but I don't think it's necessary. It's more fun for all players when they're undoctored anyway.

Also, with one particular DM, we had a problems with critical failures.
I try and force the jammed door.
/roll strength.
You fail to open the door, furthermore you have a heart attack and die. "WTF HAX!"
I boost the halfling above my head, then hold him steady so he can see.
\roll strength.
/roll dexterity.
After successfully boosting X, you drop him and he breaks your neck and you die.
Seriously people, just drop the halfling, not every critical failure has to result in death. With this guy, even intelligence rolls resulted in death. Some more I can remember are:
After showing the Balor your strength and how you and your friends could assist him, he laughs in your face and *slaughter commences*.
