Search for the 'Generating Threat' topic in the post. It lists some of the best threat skills and some priorities. The info is a bit out-dated but it's OK for a start.
I myself have macroed Shield Block and Lavanthor's talisman to my shield slams so that they will automaticaly be activated if ready just before I use SS.
For AoE threat thunderclap, shockwave and glyphed cleave are your friends. Use those and then weave the other high threat skills into your rotation when the first are on CD.
As for gear Trolls is pretty much correct in his priority. I'd suggest that you get some tanking bracers and shoulders (get some Tempered Saronite crafted) and loose the pure defence gems (+10 defence rating / 15 stamina is still good for yellow sockets).
However, if you need to increase your threat generation you can choose some BV, expertise and hit gear over dodge and parry. A lot of tanking items have expertise and hit. Lavanthor's talisman is good for threat, but your pocket watch is not really good. The emblem of triumph thrown is good too. Titanium weapon chain is good for threat. ToC 5 man has some more threat items (boots, ring, shoulders, amulet, helmet)
