This achievement is for the Kyrian special covenant feature, the Spire of Ascension.
To obtain this achievement, you must defeat Mad Mortimer on any difficulty after being afflicted with Fiery Veins, Frozen, and Uncontrolled Vomiting.

Ortim will constantly cast Burning Glob, which applies a stack of Unstable Stimulant. At 10 stacks, you will be afflicted with Fiery Veins.
Imer's Blistering Pool will apply Plague Fever every second that you stand in them. At 10 stacks, you will be afflicted with Uncontrolled Vomiting.
Mort's Freezing Ground will apply Freezing Limbs every second that you stand in them. At 10 stacks, you will be afflicted by Frozen. Standing in this will also instantly remove any stacks of Unstable Stimulant and prevent the application of Plague Fever (which will cause them to be removed after they expire).

How I went about this achievement was to try to get all three debuffs while only Ortim was active.

First, I stood in the frozen ground around Mort as soon as the battle started to become frozen.
I then stood in Imer's blistering pool to start vomiting while also charging up stacks of the unstable stimulant to be able to get fiery veins around the same time. During this time I started attacking Ortim to start getting some damage off.
Additionally, once you have fiery veins, you'll start to deal bonus damage for its duration.

So long as you can keep yourself alive in the beginning, the fight can continue as normal once you've acquired all three debuffs.

As a final note, if you for some reason take too long in the beginning, the initial pools around Imer and Mort will despawn after about 5 minutes.

You can stand in the slime puddle then the frozen puddle before aggroing Ortim to gain your 10 stack debuffs for the achiev, then just engage Ortim, get your 10 stacks of the fire and proceed to do the fight as usual and kill it. It still gave credit and I received the achievement.

I got the best way to CHEESE it.
Take any character you like.
Personnaly, I took Pelagos, cause his immune is awesome when the criteria are met (Right image of the Confront Memories)
And take the Phial of Serenity item.

Start at right for the Blistering Pool, opposite of Ortim just dip your feet at the edge of the pool.
If you do it correctly, you'll got stacks without even pulling Ortim !
After 10 Stacks, you get Uncontrolled Vomiting
1 step out of 3 done

Afterward, do the same trick with Freezing Ground
2 steps out of 3 done, without pulling Ortim !

*Keep in mind the Plague Fever will refresh itself on you until you go to the Freezing Ground, that's why you start with the Blistering Pool and not with the Freezing Ground*
Theres why the Phial of Serenity come in play.
You did the 2 steps above right ?
And you didn't pull Ortim ?
You can use the Phial of Serenity every 45 seconds to put your HP at 100% !

After that, you have to do the fight normally, BUT, without casting the immune shield of Pelagos. Just take the casting buff (middle one of the Confront Memories)
Keep an eye on your Unstable Stimulant, always be in sight of Ortim to be sure he'll always cast on you and avoid walking in the Freezing Ground or it will remove your Unstable Stimulant.
When you reach the 10 stacks and get the Fiery Veins debuff, you can start immune .
You have completed the requirements and now you can BURN HIM DOWN.

It's sure not the fastest way to do it, but it's the easiest way !

This can easily be done during the fight as you can do it on courage difficulty. I would recommend the phial of serenity for health and keeping your shield up with pelagos.

You need to complete this on a level 60 character.
As of Dragonflight, any character leveled beyond 60 (68 at the time of testing) cannot complete this achievement.
Frozen and Vomit is simple enough, but the Fire thing requires you hit by a flame attack ten times in a row.
Being above 60 (68 at the time of testing), you will simply resist it frequently. And missing a single application means it times out and falls off.

I can confirm this does not work on a geared and level 70 character. I tried party sync like suggested, and it would still miss me. Looks like this is going to have to be done in another covenant.... thanks blizzard.

Seems as of dragonflight this broken for at least level 70s as fire debuff will never be applied to you

While it seems as of dragonflight, max level characters above lvl 68 will cause this achievement to not be available to do because of how the attack constantly misses, I am happy to report this achievement is still doable.

I tried using party sync with a player that was lvl 60 and I got reduced to lvl 60 as well, he parked himself at Bastion and I did the challenge on the lowest difficultly. I managed to do the strat where you stack up the first two by dipping your toes in without aggroing, then pulled the main slime. Once I got ten stacks of all three, I bursted them down and the achievement popped.

Try party sync with at least another lvl 60 in order to get this achievement if you are at lvl 70. I can confirm this method works at least in the way I did it.

Alas, dear adventurers.

As I have read through the comments, I see people have found a way before with Party-Sync but I can inform that I attempted this today, 6th of July 2023, that this method does not work (at least not for me).

Update: Seems that the specific measures of the comments were accurate. I just found it weird that it needed to be so specific. GROUP UP WITH A LEVEL 60 IS A DEMAND, NOT AN OPTION, FOR PARTY-SYNC!

I have tried the following methods:

Using a combination of all the comments here, I completed this today using the following:

Party sync with a level 60 (they can be anywhere)
Choose Pelagos
Try on the lowest difficulty (Courage)
Use the equipment Phial of Serenity

Stand in the green pool (plague) first, then frozen, all while waiting to heal yourself so you don't die due to the damage. This will take more than 5 minutes and the pools below the Imer and Mort will despawn. It's optional to wait to engage the Ortim (the red slime) because you can use Pelagos' Confront Memories (shield) and it won't affect the stacks needed for fiery veins.

09/04/2023 - Achievement completable

1: Get a level 60 friend and get him to bastion near Elysian Hold and party-sync.
2: Make Phial of Serenity and start the Mad Mortimer fight.
3: During the fight firstly stay in the green puddle to get the 10 stacks of the Disease after that you will get debuff than stay in the frozen area and get 10 stacks of the debuff to get after that use the Phial of Serenity to heal everytime you can
and finally just let Ortim hit you 10 times with Burning Globe to get the debuff.

After that go buy your mount :)

Hope this helps atleast until Blizzard patches it.

EASY WAY TO DO THIS as long as you know how to make a trial account.

Create a trial account and launch both accounts on the same computer.
Make a level 1 character and DO NOT send them to Exile's Reach.
Travel to them on your level in 70(you don't appear to be able to look up lvl 1 trial accounts in the social window).
Invite trial account to party.(you may have to add each account as a friend before u can do this)
Open quest log/map, at the bottom of quest list is level sync.
Click sync on main and then accept on the trial account.
Your main should now be level 60. Return to Bastion.

Enter the trial, select Pelagos, select phial of serenity, choose mortimer on lowest difficulty and start.
Make sure to stay as far away from Ortim as you can as you don't want to aggro him yet.
Stand in the edge of acid pool and wait to 10 stacks of the debuff. Start casting phial of serenity on cooldown.
At 10 stacks you'll be stunned and wander around for a few secs, hope you don't aggro Ortim and then carefully make your way around to the edge of the ice and stand in it until 10 stacks. Makes sure to watch your health!
After 10 stacks you'll be stunned for a few secs. Get out of the ice and heal up a little. Ortim doesn't do too much dmg but it helps.
Pull Ortim and let him cast his spell until it gains 10 stacks and turns into Fiery Veins.
KILL and done.

as of today this acheive is still bugged for me - i can get frozen and uncontrolled vomit easy but fiery veins no luck - i raised a ticket some time ago and response was this still hasnt been fixed :( i needed this for last mount which i cant get

If you're doing this in 2024 as a level 70, just party sync with a friend - I synced with my husband's level 14 rogue - and choose Pelagos and Phial of Serenity. You can get the achievement on the easiest difficulty.

Walk over (without letting Ortim aggro you) and stand on the edge of the disease pool until you get 10 stacks. After being briefly disoriented, use the Phial of Serenity to heal up
Start using Pelegos' shield/invuln ability (3) on CD to help keep you alive long enough between Phials
Walk around carefully - still not aggro'ing Ortim yet - and now stand on the edge of the frost pool until you get 10 stacks. After briefly being disoriented, it's time to aggro Ortim
Aggro Ortim and keep using your invuln ability (3) on CD and Phials as they are ready. Burn down the boss while avoiding obvious floor swirlies
Profit!

Allright y'all, first comment ever, but I hate the idea of anyone HAVING to do this whole Path of Ascension twice for one achieve and mount. I didn't test a whole bunch of things different because I was getting the errors everyone else was getting. Loading screen then error, and not able to party sync not available, etc. You can also get this mount with the other covenants with the other covenant features if you don't care about 5 achievement points. For those of you who haven't tried to do this at level 70, the fire buff the blob is shooting at you won't ever hit, and you need 10 stacks to proc Fiery Veins. That is why there are guides for this now.

What works for sure as of 10.2.5:
1. Making a level 1 of your same class and race and server (I had my gf make one on her account). No WM, no Exile's Reach. Right to Elwynn Forest. Invite the level 70.
2. Get in front of the NPC's, party sync from that lowly level one, he has not moved at all, just chilling at the first spawn.
3. Party sync will put you right at 60, which makes this fight challenging again.

Eliwynn put it great down below but to keep it short:
1. Phial of Serenity as your equipment.
2. Choose the easiest Mad Mortimer.
3. Get the ice and puke buff WITHOUT pulling the blob. (Stand on the edge of the circle)
4. Heal up every 30 seconds to top yourself off.
5. Fight the blob, ignore the charging guys, try to keep him near the stairs, it'll save you from a couple puke swirls.
6. Don't LOS the blob, and you should get 10 stacks as long as you don't get any other buff, and have the blob low.
7. Live out the fire stun, then kill them.

Profit: All The Colors of the Painbow and the lock off the vendor to buy the Gruesome Flayedwing for 2500 anima.

If you are attempting to party sync and you receive the error "Instance Not Found" it's because you have to turn Chromie Time OFF on your lower-level character. I figured it would be the other way around, but it needs to be "present time".

luckily my kyrian with the building done was still 61, was able to get stacks of the various debuffs without a problem

Since this achievement is impossible at 70 due to spells missing or being resisted.
It would be easier if they took out the fire part of the achievement than to rework anything else.
That or made it so it doesn't get resisted.

I'm sure it's already been mentioned somewhere, but party sync with a level 1 will work for this. I used a second account that didn't have a subscription or anything, level 10 DID NOT work, but I made a level 1 toon and invited from my main account, promoted the level 1 to leader , activated party sync from the level 1 toon (don't know if this matters but it's how I did it) and I was able to enter the trials and did the achievement on my level 70. Good luck all!
