Inside the Theater of Pain in Maldraxxus (not inside the dungeon) you'll find the Sword Sorrowbane stuck in the ground and Oonar's Arm attached to it. Both of them are clickable objects so try to grab them! Too weak? Then obviously you'll need to buff your strength!

Acquisition:

Do the prequests to unlock Au'larrynar's vendor option, buy a Potion of Unusual Strength and keep it in your bags.
Buy a Strength of Blood from Ta'tru in Revendreth and keep it in your bags.
Make sure World Quest A Few Bumps Along the Way is active and you have plenty of time to complete it.
Buy a Scroll of Teleport: Theater of Pain from One-Eyed Joby for 10 Gold or keep your Hearthstone off cooldown and bind it to the Innkeeper close to Theater of Pain (maybe check the location of Sword & Arm inside the arena if you haven't seen it before)
Complete 2x ballista repairs for the WQ without losing the first buff (10min real-time, so do not log out) to get 2x Battle Hardened (second stack will refresh timer).
Head to Glutharn's Decay at far east of Maldraxxus.
Click 4x Edible Redcaps, then instantly use your Scroll of Teleport: Theater of Pain or hearth back to Theater of Pain.
Hurry up (80% slow) to get to Oonar's Arm within 2min (maybe you wanna use a Goblin Glider Kit on top of the stairs when using the Hearthstone).
Use your Strength of Blood and Potion of Unusual Strength now.
Within 25sec click Oonar's Arm for Oonar's Arm and Sorrowbane to receive Sorrowbane and Harvester of Sorrow achievement. Congrats!

There are several buffs and their combinations that affect the text messages for pulling the Sword:

Looks like Battle Hardened is only available when A Few Bumps Along the Way is active in Maldraxxus' "The Spearhead" area. You have to defend damaged artillery by defeating NPCs close to it (filling up a progress bar) and get this buff when a ballista's repair is finished. (The WQ is completed when 2 ballistas are repaired and I don't think it's possible to help on another repair, so not sure why it can stack up to 3x.)
Edible Redcaps are small mushroom objects you can click. You can have up to 4x stacks of this buff active but keep in mind that each stack also applies a 20% slow. The only ones I found are in Glutharn's Decay in the eastern part of Maldraxxus together with Dormouse Ecila. This shroom area is also the location of the Rare Spawn Deadly Dapperling (source of Shy Melvin battle pet).
I had to bind my Hearthstone to the Innkeeper near Theater of Pain or use Scroll of Teleport: Theater of Pain to get to Sorrowbane within the 2min buff duration.
Explosive Strength is the effect of a clickable vial on a table in House of Plagues area in Maldraxxus. For combining buffs I don't think it's a valuable option. I am playing a Shaman and had to click the Redcaps, hearth back to Theater, fly to Plague Watch, click the vial and Astral Recall back to Theater. Even with the now doubled duration I can only think of Shaman, Vulpera (Make Camp/Return to Camp) or Warlock teleports being able to move that much within 2min, so not a match for an item you can carry until you want to use it. And there's another problem... you don't know if someone else clicked the vial and triggered the ~5min respawn timer.
Potion of Unusual Strength is the effect of Potion of Unusual Strength that is sold for 1 Gold by Au'larrynar in Maldraxxus (outside of the eastern Theater of Pain walls). You'll need to do some quests close to his position to unlock the vendor option.
Strength of Blood is the effect of Strength of Blood. You can get them from Ta'tru in exchange for a specific amount of a Shadowlands crafting reagent (changing weekly) at the Night Market in Revendreth.

Sorrowbane (ilvl 180 at level 60) is a sword stuck in the ground inside the open world Theater of pain Arena in Maldraxxus.
You can't pick it up and each time you use it, you hear: "You lack the strength to move it."
To be able to loot the treasure you need to get several buffs

Buy 2 potions, don't use them yet
- Strength of Blood from Ta'tru vendor in Revendreth, north of the Wanecrypt Hill windrider
  Price: 30xRaw Seraphic Wing or 10xWidowbloom or similar random crafting material, changes each week
- Potion of Unusual Strength (1g) from Au'larrynar vendor in Maldraxxus, (unlocked after Side Effects Theater of pain questchain)
- WAIT FOR A Few Bumps Along the Way WORLD QUEST (west of Theater of Pain in Maldraxxus)
- Bind you Hearthstone to Slumbar Valorum Theater of Pain windrider in Maldraxxus
  Or buy Scroll of Teleport: Theater of Pain from One-Eyed Joby that can teleport you near the sword
- Complete A Few Bumps Along the Way world quest (completion of the quest gives 2 stacks of Battle Hardened duration 10 minutes )
- Run to the Deadly Dapperling rare area and eat mushrooms on the ground
  Get 4 stacks of Edible Redcap (duration 2 mins and they will slow you down by 80%)
- Use Hearthstone + Goblin Glider Kit near the stairs to get down faster
  Or Scroll of Teleport: Theater of Pain that teleports you 10-feet away from the sword
- Run to the Sorrowbane in the south-east side of the open world Theater of Pain
- When near the Sorrowbane use Strength of Blood and Potion of Unusual Strength and loot the sword

PS: WARNING Most people reported to me that the hard/ruining part is to find 4x Edible Redcap because the area is usually crowded and because Battle Hardened timer is kinda shot it leads to failing the progress. So keep that in mind and try to do this not in prime time.

To get Sorrowbane you need combine the following buffs: 2x Battle Hardened, 4x Edible Redcap, 1xPotion of Unusual Strength and 1x Strength of Blood.

How to get Sorrowbane:

speak with Slumbar Valorum to set your Hearthstone for Theater of Pain /way 50,9, 53,1
buy Potion of Unusual Strength from Au'larrynar in Maldraxxus /way 53.64, 47.91 Au'larrynar. To unlock this vendor you need completes side quests around the area until Side Effects.
buy Strength of Blood from Ta'tru in Revendreth /way 51,1, 78,8 Ta'tru. The vendor changes the price on his items every week.
complete the WQ A Few Bumps Along the Way to get 2x stacks of the buff Battle Hardened (10 min duration)
hurry to Glutharn's Decay to grab 4x stacks of the buff Edible Redcap. You get this buff after interact with red mushrooms /way 76,4, 56,7
with 4x stacks of Edible Redcap and 2x Battle Hardened use your Hearthstone to Theather of Pain
go to Sorrowbane located at /way 51,4, 48,4 use Goblin Glider Kit to reach more fast if you need

The harvester of sorrow here is blizzard, taking all the frustration and anger at the bloody mushroom spawns and unrenewable world quest buffs.

You can obtain the Battle Hardened buff while in a raid without having to complete the world quest, should you fail obtaining all the required buffs and having to wait until the world quest becomes available again.

To earn Harvester of Sorrow and obtain Sorrowbane you have to combine several buffs.
Sorrowbane can be found in Theater of Pain at 51.42 48.47 coordinates. It looks like a big sword stucked in the ground.
Unfortunatelly you can just pull of out. you arent strong enough ("You lack the strength to move it." message will be shown). So, you need to prepare.
Obtaining Sorrowbane is also required for Treasures of Maldraxxus achievement.

Step 1: Buy two potions:
1. Potion of Unusual Strength - sold by Au'larrynar, Maldraxxus, coordinates are 51.34 48.57. Price is 1 gold. Complete shorth prequest around there to unlock him.
2. Strength of Blood - sold by Ta'tru, Revendreth, coordinates are 51.00 78.62. Price changes every week. For me was 20 Laestrite Ore

Step 2: Wait until A Few Bumps Along the Way world quest appears. Malrdaxxus, around 40.96 50.22 coordinates.

Step 3: The day when A Few Bumps Along the Way is up:
1. Set you hearthstone to The Theater of Pain - speak to Slumbar Valorum at 50.92 53.09 coordinates (or buy Scroll of Teleport: Theater of Pain either).
2. Complete A Few Bumps Along the Way, you will get 2 stacks of Battle Hardened.
3. Mount up and make you way to 76.79 56.89 - Glutharn's Decay, there you need to click to 4 Edible Redcap. BEWARE of Deadly Dapperling rare mob.
4. After consumong 4 Edible Redcap (you must have Battle Hardened x2 and Edible Redcap x4) use your Hearthstone and immediately run down the Theater of Pain stairs to the Sorrowbane.
5. Get to the sword and drink both Strength of Blood and Potion of Unusual Strength. DRINK Strength of Blood FIRST! It lasts longer!
6. So, while you have all buffs - Battle Hardened x2 +Edible Redcap x4 + Strength of Blood + Potion of Unusual Strength - pull out the sword. Harvester of Sorrow is yours!

Few hints:

You cant carry more than one of each Strength of Blood and Potion of Unusual Strength so you need to be sure you use them in the proper time.
When you are gonna to consume Edible Redcap - be sure Deadly Dapperling rare mob will not disturb you. Consuming Edible Redcap decreases your moving speed and rare cast explosive mushrooms. You may not to avoid them while slowed so you die and fail the achievement.
Bring Goblin Glider Kit and use it to fly down over the stairs in The Theater of Pain. This may help you to save few seconds.
If there's a fight on the Theater of Pain you MUST BE CAREFUL. Dont use potions until you are next to the sword and able to touch it. If someone fights Unbreakable Urtz it will surely engage you into the fight so you have to be sure you will not be thrown away, stunned or charged while pulling the sword out. If Unbreakable Urtz charges you while you are slowed and can run off its aoe, well, your are dead. Well, its actually a bit of luck requires.
There is also Oonar's Arm nearby, you can pull it out as well to get Oonar's Arm.
Obtaining Sorrowbane is also required for Treasures of Maldraxxus achievement.

If you aggro Deadly Dapperling after obtaining the last Edible Redcap just walk away.
Mr Dapperling will not follow you for long.

So if you wanna do this in a party or something. Do not! There's not enough Edible Redcap and if you relog - your Battle Hardened will be gone. Learnt it the hard way.
So you're getting punished for doing content with your friends.

Okay don't do what I did, I thought I was being a clever Necrolord by using the Sixfold Recluse portal to then get the portal to the House of Rituals to quicker get to the edible redcaps after getting my battle hardened buff....a little too clever for my own good it seems because using the portal rids you of your buff. And I had already completed the WQ so now I will have to wait until it pops up again. UGH.
