This one is pretty straightforward, and in gear should be an absolute breeze.

Every time the boss hits 100 energy she casts Soulrend, spawning an add from every player in your group except your tank. These move towards her and do some raid damage if they hit her. For the achievement, one player needs to stand next to each of the 4 brazier in the corners of the room. Once your add spawns the brazier should be lit AF. Everyone should just then kill their own add, then go help the healer with theirs, while the boss stays near the middle to give more time for each adds. Our ~295 Ilvl DPS had no problem killing their own adds in time, so you shouldn't have to worry about them getting in.

Only other thing worth mentioning is that it appears the flames go out after a certain amount of time (30+ seconds, past that can't be more specific) so you want to get her low before you light the braziers. We overdid this a little and had her at about 20k health when the adds spawned, so we just killed her and ignored the adds and still got the achieve no problem.

In order to lit the Shadowflame Sconces, everyone in the group except the tank will have to spread and stay near one when the boss casts Soulrend. The sconces will stay lit up for about 25 seconds, so you should do this when the boss is low on health around 5 to 10%.

Our group did this at 5% or less and at the point at which we were sure it would be the FINAL soul rend of the fight. Damage can be quite high at this point so everyone must be on their toes with defensives, and burst down their add then the boss to death after lighting the braziers (and within 25sec from lighting). We also confirmed that each brazier WAS lit as you have to be face hugging it to light it (just being close is not good enough). ilvl of the group was about 342.

Since I'm one who often doesn't have access to many people to do achis, thought I'd answer a question I had before doing this: it's doable in 4 people, doesn't require 5, at least without having anyone in tank spec, it spawned adds from everyone.

This achievement CAN be soloed, even if it's quite tedious. It also requires fast loading time, disabling all addons helps because when you disconnect (not using Logout) it will take a while before the character goes offline, and that's the mechanic we'll use for this!

You need 1 character that can burst down the boss from 100% to 0% fast (I had a DH with ~395 ilvl) and 3 other characters that can enter the dungeon (50+ I think, I had lvl 60 characters). You can use the group finder to get them in the same group. Yet again, make sure you disconnects them and not "logout", that might disband the group. (I used 2 windows of wow so I could reconnect on one of them to disconnect the other window).
Place one character at each braizer before pulling. Log on to the first character, as fast as it's logged on, reconnect on the other window and log on to the next char. Log on to your main character last and pull the boss. Keep rotate logging on all characters till boss casts Soulrend and lights the braizers. When you get back on your main character and braizers are lit just burst Yazma down.
